The boys are playing with pencils before class - Beavis tries to stab between his fingers, and Butt-head sticks them into his nose and ears. Mr. Buzzcut starts class by introducing the new school psychiatrist, Dr. Floss. She invites the very bored-looking class to come see her if they have any problems. She claims her services will be offered on a voluntary basis, but when she sees the boys playing with the pencils, she orders them into her office.

Dr. Floss has the boys lying on her couch, and urges them to tell her their earliest memories. Butt-head describes a time he had to take a whiz, while implying his penis was huge, and Beavis just giggles. Then Dr. Floss wants the boys to describe how they feel about their mothers. Beavis makes hand gestures as if he was feeling around a large body. Butt-head insults Beavis, who slaps him off the couch. Butt-head sees a set of colliding balls, and holds it up in front of his crotch, while saying "ow".

Beavis takes the only grape lollipop out of a jar on Dr. Floss' desk, and Butt-head decides he wants it too. They begin fighting for it, and then Beavis spits on it to coat it in his germs. They continue to fight, as Dr. Floss records notes about them in her notebook.

She gives them an inkblot test, and in each picture, the boys see masturbatory images and use vulgar language to describe them. When she shows them a very suggestive-looking photo of a singer holding a microphone, the boys see only a bunch of "weird shapes". Dr. Floss calls in the principal, sounding concerned.

In the final scene, the boys are in a psychiatric ward with a bunch of mental patients.
