Ezekiel Stone wakes up on a subway train in New York City. Going up top, he takes in the sights like a tourist, then goes to a church and enters the confessional booth. Stone starts to talk to the priest, saying that he killed a man. Fifteen years ago he was a cop whose wife was raped, and Stone tracked down the rapist and killed him, making it look like a drug overdose. Stone explains that two months later a thief gunned him down, and he went to Hell for the sin of murder. However, six weeks ago 113 inmates escaped from Hell, and the Devil has sent Stone to track them down.
The priest claims to be puzzled until Stone identifies him by name as Father Edward Salinas and says that the priest knows what’s going on. Salinas then rips his way through the side of the confessional and out into the street. Stone draws his service revolver and follows, trapping Salinas in an alleyway up against a wire fence. However, police detectives Kane and Hirsch arrive and arrest Stone, while Salinas seemingly burns his way through the fence with his bare hands.

Time to give the Devil his due, Father.
The detectives believe Stone is the kidnapper of two missing altar boys, figuring the priest saw him and Stone tried to eliminate the witness. While the older Hirsch goes after Salinas, Kane explains what’s been happening. Once Stone confirms there are two altar boys missing, he breaks Kane’s nose and escapes.
Stone goes to a hotel and gets a cheap room. Going in, he examines himself in the mirror and discovers he’s covered in tattoos in some ancient language. He finds the Devil waiting for him on the balcony. The Devil wonder if he chose the wrong person to track down the inmates. When Stone complains about Salinas having super-strength, the Devil explains that the longer someone is in Hell, the more of it they absorb into itself... and some of the prisoners have been there since the dawn of time. However, the Devil assures Stone that since he and the inmates are already dead, they can hurt each other but no mortal force can damage them. When Stone asks how the inmates can be sent back, the Devil finally explains that the eyes are the windows of the soul: if anyone destroys the eyes, mortal or immortal, the soul goes back to Hell.

God’s universe doesn’t work like the American legal system. You do something, you pay for it.
Getting one straight answer, Stone asks for another: how will he get his second chance? The Devil says that’s for him to know and Stone to find out, but that Stone will have to return all 113 inmates before he gets it. When Stone points out that the Devil has no power on Earth and is covering his ass, the Devil simply shoves him off the balcony where Stone lands below, unharmed.
Kane and Hirsch identify Stone from the artist’s sketch and talk to Tibbetts, Stone’s former captain. Tibbetts confirms that Stone was a decorated cop who died 15 years ago.
Salinas has gone to a museum and chats with a teacher who has her students on a field trip. Salinas gets one of the boys in the restroom and confirms he’s an altar boy. The priest tries to lure the boy to the zoo but when the kid refuses, Salinas grabs him. A man comes in and tries to stop him, and Salinas grabs his face, burning him.

Kane tries out his theory on Hirsch: Stone’s marriage fell apart after the rape of his wife Rosalyn. Stone killed the rapist, then faked his death to avoid an IA investigation. Hirsch since is skeptical, since there’s no reason for Stone to come back now and chase Salinas.
Stone goes back to the church and talks with the head priest, Father Horn. Horn is blind and talks about how he was shot by a husband when he interfered in a domestic dispute, but still believes in God’s justice. Stone is a bit more skeptical, and finds some coins while going through Salinas’ room. Kane shows up and tries to arrest Stone. Horn doesn’t believe in Stone’s guilt and distracts Kane so Stone can escape to the roof. He jumps to the next building and Kane follows, but misses the jump and hangs from the rooftop. Stone reluctantly pulls him up and explains that Salinas is obsessed with the Book of Revelations. He believes that to fulfill the prophecy of the four great Beasts, he must find them in altar boys and then kill them to send them to Heaven. However, Kane is skeptical when Stone explains that Salinas killed 16 boys in Italy... in 1896. Further, Salinas then moved to New York City and continued killing until he was captured and executed. Stone doesn’t care if Kane believes him and walks off. When Kane tries to shoot him in the legs, the bullets have no effect and Stone jumps off the roof, falls five stories, and casually walks away.
Kane and Hirsch are called in to investigate the third kidnapping. They find the dead man, with Salinas’ handprint burned into his face. The CSI tech also finds a bizarre substance all over the restroom: a mix of human sweat and sulphur. Kane begins to wonder if Stone was telling the truth.

You go your way, and I’ll go Yahweh.
Stone tracks down a coin collector who identifies Salinas’ coins as subway tokens from the 1900s. He goes to a museum and gets information on the subway systems of the 1900s from an attractive woman. She asks him out to dinner but he turns her down, claiming he’s married. He then goes to his old house where nobody currently lives, but Stone can remember when his wife was there.

Kane has checked the files and found a newspaper photograph of Salinas: a photo almost a century old. Salinas was hung in 1906 after ranting about the Book of Revelations.
Salinas is hiding in the subways and has the three altar boys tied up. He puts a mask of the Beast on each one, and prepares the fourth one for his next victim.
Using the curator’s information, Stone finds the old subway line. He finds the three missing boys and tells them to get to the surface and ask for Kane. Kane gets word and he and Hirsch meet the boys. Kane takes care of them and confirms Salinas was their abductor, while Hirsch goes to find the priest. As he leaves, the boys say that Stone was the one who saved them. Hirsch finds Salinas, who impales him with a cross. He then attacks Stone, ranting and raving about how Salinas is the hand of God and Stone has been sent to stop him. Salinas is able to hurt Stone, but Kane arrives and shoots him. The bullets have no effect so Kane tries to grapple with the priest. Salinas slams through a brick wall and into an adjoining subway line. He throws Kane off... and a subway train hits him. He’s unaffected and prepares to kill Kane. Stone calls out and when Salinas turns, Stone shoots him in the eyes. The energy of Salinas’ soul pours into the air and then is sucked downward.

Later, Stone and Kane talk about what happens, and Kane admits it’s affirmed his faith in God and justice. He also tells Stone that he did some checking and Rosalyn moved to California and is still collecting his death benefits. Stone wonders if he would have killed the rapist if he had a second chance, then walks off into the night.
