After Buffy is attacked by a demon as she gets close to the Trio’s lair, she begins experiencing hallucinations of herself in a mental hospital with her still married parents looking out for her. Initially, dismissing them, the recent burdens of her life and her own history in a mental hospital make Buffy question whether she is the Slayer or if she's still in an institution.

This would be the last time Dean Butler is seen as Hank Summers, Buffy's always absent father. Kristine Sutherland also makes an appearance as Joyce Summers in the alternative reality.
Though it is seen again and for the final time in "Entropy", this marks the last time we see Buffy working at the Doublemeat Palace.
Buffy: I was in an institution. There were, um, doctors and nurses and other patients. They told me that I was sick. I guess crazy. And that, um, Sunnydale and all of this, none of it was real.
Xander: Aw, come on, that's ridiculous. What, you think this isn't real just because of all the vampires and demons and ex-vengeance demons and the sister that used to be a big ball of universe-destroying energy?
Spike: Oh, balls. You didn't say he was a Glarghk Guhl Kashma 'nik.
Xander: 'Cause I can't say Glarba --
(after tranquillising the demon)
Xander: I altered his reality. Get it? I -- Never mind.
