At the Haven psychiatric facility, Dr. Lucassi and Gail are working with two of the catatonic patients, Howard Sperry and William. Lucassi works with one of the patients on his own, and the staff hears a crash. They discover that Lucassi is standing over a spilled tray of medicine. The doctor goes berserk and attacks an orderly, Morgan, throwing him into the wall with superhuman strength.
Audrey is settling in to her new temporary job as a Haven cop, and Nathan figures she'll stay so she can discover if the woman in the photo is her mother. She admits that she thought the cases would be more troubling, and Nathan warns they'll see more such cases as the troubles continue. They get a call to check out the psychiatric hospital and go to investigate. They discover that Lucassi is on a rampage.
Nathan and Audrey pursue Lucassi into the facility and manage to subdue him using either. Afterward, they determine that three of the patients are missing. As they leave, Audrey figures it's more of the Haven crazy, and Nathan figures that she hopes it's something weird. Audrey insists that her interest is her mother's disappearance. They get word that two of the patients have been spotted stealing clothing and go to the house. They discover that the two patients are playing with a boy. William is on the trampoline playing and Howard is laughing and talking normally.
Ray comes home and discovers that Lilly is checking the refrigerator, seemingly normal. She has no memory of being committed and a happy Ray is glad to kiss her.
As Audrey and Nathan bring William and Howard back to the facility, Howard starts reciting flower names. He then becomes comatose and William is similarly afflicted. As the medics bring Lucassi out, he recovers his sanity and diagnoses them. Audrey releases him and Lucassi explains that both men are catatonics. They tell him what Lucassi was doing, but he doesn't believe his drugs could have caused such a reaction. He warns them that Lilly is dangerously violent. As they go to the McBreen house, he explains that Lilly has an obsessive disorder. A musician, she gets destructive when she feels her music is escaping her. Ray returns and thanks Lucassi, and insists that Lilly is fine. However, when they go inside, they discover that she's torn up the place. Ray figures that she'll go to look for a piano to compose music. Audrey and Lucassi go to test his drug to see if it caused the outbreak, while Nathan and Ray go to a local bar.
As they arrive at the bar, Ray explains that Lilly was approached by a record executive who wanted her songs. He drove her up to her favorite composing place, got distracted, and went off the road and into the river. She was trapped underwater for eight minutes, and only the extreme cold kept her from suffering brain death.
As Lucassi works, he explains that he lost a relative to Alzheimer's and his research is intended to cure the disease. He recreates the exact dosage he spilled and then tricks Audrey into going outside of the lab. He then locks her out, monitors himself, and spills the drugs. However, nothing happens this time. Audrey notices the surveillance cameras and gets an idea.
Duke is leaving the bar with a mounted fish. Nathan wonders if he's up to something and Duke insists he's just decorating his boat. Nathan gives up the issue for now, and Duke complains that Nathan is harassing him. As Ray goes inside, Duke asks Nathan if Audrey knows that he's not a real boy, and about the things he's done. Before Nathan can respond, Ray tells Nathan that he's found Lilly inside.
At the station, Audrey and Lucassi watch the footage of the spill. She notices that William was non-catatonic before the spill. She calls Nathan and tells him what they've discovered. Nathan tells her that they've found Lilly, and she's reverted again. She's unable to play the piano, and Ray tries to accompany her.
When Audrey and Lucassi arrive, they discover that the bar patrons have gone insane. Nathan is holding an open flame to his arm but doesn't feel anything. He shoves her aside and leaves.
The police round up everyone except Nathan, while Lucassi suggests that Lilly is the common denominator. Audrey gets a report that Nathan is at the marina.
Duke is unloading his boat when Nathan arrives and asks him what the sun feels like. He starts smashing the cargo and then punches Duke. He starts choking him but Audrey arrives in time to taser him unconscious. She asks Duke to keep an eye on Nathan while she goes to stop Lilly from further spreading the insanity plague. When Duke hesitates, she reminds him that she kept him out of jail and he reluctantly agrees. As she goes, she calls Lucassi and confirms that Lilly was playing the piano both times.
Duke chains up Nathan, who wakes up in a frenzy. He accuses Duke of being a parasite who uses people. Duke accepts his insults until Nathan accuses him of planning to use Audrey, and then hits the policeman.
Audrey searches Lilly's room and finds a photo of Ray and Lilly on a boat, the [em]Caprice[/em]. There's no trace of the boat, and Lucassi confirms that the boat belonged to Ray's grandfather. Duke calls to tell Audrey that he's done taking care of Nathan. Nathan has regained his sanity and Audrey comes to collect him. She explains her theory about Lilly playing the piano, but Nathan tells her how Ray played the piano with Lilly.
At a boatyard, Ray prepares the boat for their trip. She asks if she drove the men insane at the bar, and realizes that he did it. Lilly tells him that he can't keep doing it.
Audrey remembers seeing Ray on the surveillance tape and figures he didn't know what he was doing at the time. Nathan realizes that Ray now knows what he can do.
Ray insists that he won't hurt anyone, but he doesn't want to se her become insane again. As she turns around, Ray unloads several musical instruments from a bag.
Duke offers to take them to the boatyard, but Audrey doesn't want him to go along. When Duke insists, Audrey wonders why and he admits that he wants to know what is going on.
Ray finishes repairs on the boat and discovers that Lilly has reverted. He starts to play the piano, unaware that Lucassi is there trying to locate the boat. While Lilly recovers, the doctor goes insane and comes after them.
Audrey, Nathan, and Duke arrive at the boatyard, and Audrey insists that he stay at the truck. As they approach the boat, Audrey notes that Nathan recovered more quickly than the staff at the facility, and wonders why. Lilly runs up to them and explains that Lucassi captured Ray and took him to the facility. They drive there and Audrey has Duke watch Lilly while they go inside. They discover that the crazed Lucassi has sedated Ray and is preparing to remove his brain in hopes of finding a cure. Audrey manages to get close enough to use defibrillator paddles to shock Lucassi insane. She explains that she realized electricity could cure the insanity when she used her taser on Nathan.
Lilly and Ray are reunited and Lilly warns him that she's changing back. He tries to help her again but she insists that it's not worth the risk and makes him promise not to try. Lilly starts singing and then loses her sanity again as she loses her song.
Back at the facility, Ray explains that he stopped playing when Lilly got sick. When he played the piano at the facility, he remembered what he could do. His grandfather came to Haven on the boat, and told Ray to always stay from music. Ray figures that his grandfather knew what he could do. Audrey suggests that a small boat on the ocean might provide the necessary solitude to avoid endangering others. Lucassi is reluctant but Audrey believes that letting Ray and Lilly go off is the best thing. The doctor agrees and Ray goes to Lilly's room to cure her. They then take Lilly, Howard, and William to the boat and Ray prepares to sail away. He plans to return once they think it's safe, and maybe help some other people. As they start to go, Howard says that Audrey is the spitting image of a woman he sold flowers to. He finally remembers the woman's name: Lucy.
As Audrey watches the boat leave, Duke asks her not to tell anyone he was a good citizen. She thanks him for helping and Duke admits he's glad that he could help her. he then asks her if the troubles are back, and she admits that they are.
Audrey tells Nathan what she's discovered, and she admits that the weird stuff does turn her on. When she says that she just wants to fix people, Nathan wonders if she'll fix him. Audrey passes off the comment and Nathan gets a call telling him that three mental patients have disappeared...
