Dr. Nathaniel Grant, a lung and heart transplant surgeon at St. Jude Regional Transplant Center in Pittsburgh, deals with working with his ex-wife, Kate, as they work around the clock to successfully harvest and transplant vital organs to those who desperately need them.

Grant, now the chief of staff at the transplant center, considers a son's request to give part of his liver to his dying father. Meanwhile, Kate hopes to harvest organs from an unidentified homicide victim; an arrogant new doctor slips into Grant's old job; Simon performs his first solo surgery; and Thea stresses out over an approaching dance party.

Simon makes a serious mistake during transplant surgery, rendering a new liver ineffective. But another liver may become available via a living donor---the patient's brother, who is incarcerated for murder. Meanwhile, Grant forms a bond with a teen suffering from transplant complications.

4 :01x04 - Mother & Child Reunion

Dr. Jonas prepares to perform the first uterine transplant, but first must get Dr. Grant's approval. However, Dr. Grant has other things on his mind as he races to save a six-month old baby with a rare heart defect and is reluctant to give Dr. Jonas the go-ahead on the risky transplant. Meanwhile, Dr. Grant has an opportunity for a uterine donor, but Kate has to get through to the grieving mother in order for her to consent to the donation.

Jonas' immediate focus is to perform an intricate multi-visceral transplant, a procedure Grant originally rejected. Elsewhere, Simon desperately works to impress the staff in light of the imminent end of his fellowship.
