Scott and Annie are having dinner when Scott throws a roll with his left hand at an obnoxious man at the next table. Scott doesn’t seem to notice he’s throwing anything, then complains that his food doesn’t taste spicy despite his wife’s observation that it is. The man from the next table comes over and accosts Scott, who dumps a glass of ice water on his crotch. Scott denies that it’s him and then collapses, a bloody tear dripping from one eye.

House wakes up in his apartment, smiles, and then goes to the bathroom where he finds Cuddy’s lipstick on the sink and notices in the mirror that he has lipstick on his face.

Taub talks to Cameron about Scott’s case and asks about her wedding. She says there’s been a glitch in the wedding and insists House will be interested in Scott. She explains that he underwent surgery for seizures and the result was a divide between the left and right sides of his brain. Taub talks to Scott, who says that he always finds extra items in his shopping cart. He claims his left hand is working on its own. They figure House will be interested. House arrives at the differential, singing, and warns them he’s in a good mood. Taub explains that Scott has split brain syndrome and the team discusses the potential symptoms, with House insisting the right brain is all but useful. Cuddy comes in and says they need to talk. In House’s office, Cuddy tells him that she’s his boss and they’re going to focus on their working relationship. She insists she isn’t getting emotional and tells him he can accept the rules or leave. Once she goes, House considers her lipstick.

House goes to see Wilson and tells him he’s Vicodin-clean and that he slept with Cuddy. Wilson is duly impressed, and wonders how House is going to screw it up. He suggests that Cuddy is either scared, or she made a mistake. Either way, he tells House he needs to talk to her.

Taub and Thirteen are checking Scott’s apartment and spot fungus on the ceiling. As they check, Taub asks her about the wedding glitch and she tells him it’s about sperm. Taub goes to see Chase and brings up the sperm right away, and Chase explains that Cameron is planning for failure.

House goes to Cuddy’s office and waits for her, but then takes thermal photos and leaves.

Annie brings Scott his toiletries and she tries to impress his other half. Scott grabs a can of deodorant and throws it against the wall as Thirteen arrives to give him anti-fungal meds. As Annie leaves, Scott slaps her with his left hand and then apologies profusely. Scott tries to go after her but collapses, unable to walk.

In differential, the team discusses the new symptom and House suggests it’s a meningioma that is reconnecting the two parts of is brain. They go out to test for it, while shows Wilson the thermal-imaging pictures he took of Cuddy. He notes that she’s radiating more heat after being with House, and tells Wilson not to talk to Cuddy. House insists on determining on which matter Cuddy is lying, but Wilson tells him that if it matters to him, he should be an adult.

Foreman, Thirteen, and Taub test Scott. There is one word he can’t recognize, “candle,” but he can draw. He then sees a visual message saying “Stand up” and stands up. He asks to leave but they tell him to stay. House, arriving, realizes the left hemisphere is in control. He notices that Scott is cold and scratching at his side. House goes in and recognizes that Scott’s breath smells like ammonia, and his liver is failing.

Wilson is reporting to Cuddy and she wonders if he has something else to say. Wilson says he doesn’t. House comes in and asks for permission to do a liver biopsy for sarcoidosis. She figures he’s trying to make her angry because a liver biopsy isn’t dangerous, and he says he’s not doing his clinic hours, and she calmly tells him to see what happens if he doesn’t. House takes her coffee and leaves.

As Thirteen and Taub do the biopsy, Scott apologizes for being so sweaty. His left hand grows increasingly violent, and Scott asks for music. Meanwhile, Wilson suggests that House is letting his relationship with Cuddy distracting him from his pain, and House needs to wait until Cuddy is ready. House tells him that he found oxytocin in her coffee cup: the chemical involved in emotional bonding. Wilson doesn’t believe it, noting that he’d need a baseline to compare to. House admits he paid someone at her gym to steal her water bottle. Next House plans to get her to lose her temper and get past her façade. Wilson admits that at least he’s committing himself to someone. They’re interrupted when Eugene Schwartz arrives at House’s office. Cuddy sent him to the office from the clinic. Eugene explains that he squawks and House makes a hasty exit.

Scott uses his left hand to play air guitar to the music, distracting it so they can do the biopsy. However, Thirteen notices hemorrhages beneath his fingernails and Scott starts vomiting blood. House calls the team to the locker room to avoid clinic duty. House suggests pancreatic cancer due to the sweating and orders tests.

House gets Eugene to drop a stool sample off with Cuddy as she’s meeting with contributors. Meanwhile, Scott starts to check himself out over Taub and Thirteen’s objections. He admits that Annie is tired of being insulted by his left hand and has left him. However, his left hand starts undressing him and Taub points out Scott’s right brain doesn’t want to leave.

Cameron goes to see House as he looks at Cuddy’s coffee. She wants advice on whether to let Chase go or to talk to him, but she admits she doesn’t have any new facts and Chase has too romantic a view. House tells her to destroy the sperm but Cameron isn’t interested. She admits she has normal doubts. Taub arrives to tell House that Scott is checking out. House goes to Wilson’s office and tells him they need his help. They scan his brain with the MRI while Wilson asks Scott what he thinks his right brain wants. He tries to get Scott to communicate with his right brain and suggest he talk out loud to explain to his right brain what he’s doing. The MRI turns up negative. The team is in differential and House suggests that the tumor might still be forming in the pancreas and creating clots when Eugene calls House to tell him that he’s squawking. House realizes that Cuddy gave Eugene his phone number. House insists it’s pancreatic cancer and suggests they open up his pancreas and paint it with paint and scorpion venom to find the cancer generating the clots. The procedure is dangerous but Taub notes that it could potentially kill only one side of Scott’s brain. They figure he’ll be okay with that.

As Chase and Foreman operate, Foreman offers his condolences on the wedding falling through. Chase notices Cameron outside and goes to see her. She tells him she wants to marry him and she has the forms to destroy the sperm. Scott’s BP drops and Chase goes back in, as Foreman admits they haven’t found any cancer.

Wilson goes to see House and asks again if he really wants Cuddy, or if it’s another challenge. He warns that House or Cuddy might get hurt. They look on as Cuddy ushers a pirate stripper out of her office. She doesn’t lose her temper, but House has an epiphany. He goes to the OR and tells them that clots were caused by arrhythmia of the heart when they weren’t looking. He tells them to do a transesophageal echo and they check the heart, finding the clot. Now they need to figure out why the heart is sending out clots. Thirteen suggest Cushing’s causing excessive cortisol production and House orders tests. As Thirteen and Taub run the tests, Annie comes back and suggests that his left hand reacted badly when Annie brought him his deodorant, and it threw the can of deodorant. As they go to do some research, Scott’s left hand reaches up to caress Annie.

House goes to see Wilson, pointing out there’s no lipstick on Cuddy’s coffee cup. Wilson tells House to get a grip and figures that he’s scared, and tells House to get serious and really make Cuddy scared so they can open up a line of communication.

Chase goes to see Cameron and points out she doesn’t want to destroy the sperm. He doesn’t believe her but understands she doesn’t want to let go. They embrace and Cameron admits she never cancelled her plans either.

House goes out on the lobby balcony and calls for everyone’s attention. He tells everyone that he slept with Cuddy. Cuddy storms out as someone tells her what happened. House just smiles and turns away as Cuddy glares at him and then comes after him, furious. He responds by suggesting they move in together and she laughs, cries, and then fires him. Eugene pops up and notes Cuddy probably likes him. House tells him he has acid reflux, and the acid splashed on the vocal cords causing the squawking. House tells him to go away but notices that he’s moved his belt around because it’s sore. House touches his stomach and finds some pain, and realizes Eugene has a tumor. House realizes he was obsessed with Scott’s pancreatic cancer because he subconsciously realized that Eugene had it. Taub reports to tell House that the deodorant caused Scott’s condition, including the initial seizures. House tells him to do a scan on Eugene for pancreatic cancer.

House goes to see Cuddy and asks about her lipstick. He asks if she’s overreacting to the other night and she admits it’s the final straw and they could never have a personal relationship. She explains she’s angry at him for something he said, not what they did. He thinks back to when he insulted her daughter and then asked for her help getting off the Vicodin. This time he remembers her walking away and not spending the night with him. He reaches in his pocket and discovers the lipstick he’s been carrying is actually a bottle of Vicodin. House tries to piece together everything and realizes Cuddy was never at his apartment and he’s never stopped taking Vicodin. Hallucinations of Amber and Kutner appear to say he made up a nice story, but it isn’t true.

Snapping out of it, House tells Cuddy he’s not okay. She takes him to Wilson.

Cameron and Chase hold their wedding. Cuddy arrives with her daughter, and Taub, Thirteen, and Foreman are there as well. Meanwhile, Wilson drives House to a psychiatric hospital.
