At a medieval town, knights gather before King Miles and Queen Shannon, and Shannon chooses Sir William to fight for her honor against Horace, the captain of the king's guard. William is initially disarmed and staggered, but regroups and regains his blade, and then defeats Horace. As the crowd cheers, William suddenly has a seizure. He pulls off his helmet, revealing that the whites of his eyes have turned blood red. Miles takes out a cell phone and calls an ambulance to the fairground.
House is at the condo, naked and taking Ibuprofen for his leg pain. Wilson's new girlfriend Sam comes in. House is fine, and she's briefly taken aback. She quickly recovers and explains that Wilson had to go to the hospital, and then says she'll be fine with them forgetting the whole thing never happened.
Foreman and Chase are deciding what case to take next when House arrives, wielding William's sword. He explains that William is in the ER and he lives at a Renaissance community full-time. Foreman speculates the seizure was caused by a concussion, but Chase notes that if it were that simple, ER wouldn't have called in House. House notes the blood-filled eyes and tells Foreman and Thirteen to take samples at the festival, while Chase and Taub run a MRI. House then goes to see Wilson and wonders why he didn't tell him about his new girlfriend. However, House realizes that Sam is Samantha, Wilson' first ex-wife that he never met. However, he backs off, saying he'll soon have the chance to say "I told you so."
As Chase and Taub run the MRI, they discuss William pretending to be a knight. William overhears them and insists that he and the others have a code of honor. He suddenly starts vomiting and they get him out of the MRI chamber.
As Foreman takes samples, some of the fairgoers mock him while staying in character. Thirteen tells him to relax and samples the food. Foreman gets the call that William may have food poisoning and she quickly spits out the food. Miles and Shannon come over to ask about William, and the doctors ask to see William's camp. They find vomit and a cow's eye, and Miles explains it's part of a food test, an idea he took from
Fear Factor.
Back in differential, House notes that if it were food poisoning then everyone would be sick. House tells Foreman to start performing allergy scratch tests and treat with epinephrine. Meanwhile, Shannon comes to see William despite the fact she's preparing for her marriage for Miles. William suddenly has a tachycardial attack and Foreman pulls open his shirt to reveal blisters. The team figures that it's not an allergy, and Thirteen suggests MRSA. She figures Foreman might have contaminated his hands when he was at the fairground. They put William into isolation and administer antibiotics.
House goes to see Cuddy and insists that they need to break up Wilson and Sam. He reminds her what Wilson was like when Sam dumped him the first time, and Cuddy admits it was bad. However, she figures it's better to stay out of it, and if House forces Wilson to choose, he might not like the answer.
Thirteen goes to see William, and Shannon is still there. He asks her to leave and William says that the pain in his chest is getting worse, but he didn't want to worry Shannon. Thirteen points out that he's in love with Shannon, and he finally admits the truth. However, she's engaged to Miles and William refuses to steal his friend's bride. William complains about pain on his back, and they discover pustules. Chase and Taub analyze them, and Taub thinks it's stupid to fight. Chase figures that's because Taub never won a fight. House comes in and tells them the blisters are from poison ivy, burned in the campfires. House also has it from William's sword, confirming his theory. William seizes and Chase manages to revive him just in time using epinephrine.
Back in differential, House insists that it must be an environmental toxin, causing trichinosis. He orders treatment and a muscle biopsy to confirm. As House leaves, Wilson tells House that Sam wants to know him better. He's agreed so Sam won't learn that he was trying to hide House. House agrees and Wilson insists he doesn't want House's help. He tells House to be his usual selfish self.
Shannon tells William that Miles is coming, but William doesn't want Miles to see him that way. As she takes a muscle sample, Thirteen explains that it's a guy thing. Once she's gone, Thirteen tells him that he should take what he want.
Sam and Wilson are waiting for House⦠who arrives with a transvestite prostitute, Sarah.
Thirteen tells Foreman it's not trichinosis and believes that House's diagnosis of environmental toxins is wrong. Foreman suggests a fungal infection and Thirteen warns they'll have to take all the samples. However, he notes that House is in more pain, meaning he's off his game and won't accept their theory unless they absolutely prove otherwise.
Sarah, Wilson, and Sam get along fine, much to House's dismay.
William suffers from excruciating pain in his legs, which have swollen up with blood due to kidney failure. Taub believes it's cancer but House tells them to check William's apartment for environmental toxins even though he hasn't been there for weeks. Thirteen and Chase go there while Foreman and Taub ultrasound the liver.
Wilson corners Cuddy in the cafeteria and tries to discuss House's efforts to sabotage his new relationship with Sam. She isn't interested in discussing it so he continues anyway, and admits that things are going well. Cuddy tells him to give it a chance.
Taub and Foreman find dozens of what initially appear to be tumors. Upon closer examination, they realize that they're not cancer tumors.
At William's apartment, Thirteen finds medieval miniatures and Chase locates a locked door. When he fails to break it down, Thirteen finds the key under the mat and opens it. Inside is a witch's chamber filled with black magic artifacts. They take them back to the hospital but are sure there are no poisonous substances. The black spots on his liver are cysts releasing some kind of substance into his blood. House notices the miniatures are lead and tells them to determine if the cysts contain lead.
Wilson and Sam return home and she suggests they have sex. However, they discover House in the kitchen, making supper for them. He's surprisingly apologetic and convinces them to stay for dinner. Everything goes well until Wilson goes to the bathroom. House then tells Sam that she's a cold-hearted bitch and he's not going to let her mess up Wilson again. He's just serving supper to get to know his enemy. They're both glad to know they don't have to pretend to like each other except when Wilson is there. Wilson returns and they proceed with their act.
The next morning, the team tells House that there's no lead in William's system and his heart and BP are becoming increasingly erratic. House decides to go to the fair in costume with Thirteen. She figures they should all just stay out of Wilson's business, but House warns that Wilson is too nice a guy and they have to look out for her. House smells the apothecary shop and talks to the owner's nephew. He clearly has no idea what's going on there. House finds hemlock and the apprentice had no idea. He admits that he sold it to one person: Miles.
Back at the hospital, House and Thirteen confront Miles. He denies trying to poison William, and explains that he bought the hemlock for the food challenge but had no idea it was poison. The team confirms that William has signs of hemlock poisoning. William insists that Miles is a good man and wouldn't have used poison. They discover that the hemlock antidote is having no effect, so it can't be what's killing him.
House is in his office considering William's sword when Lucas gives him the investigation file he's come up with for Sam. He admits he stole her psychiatrist's files on her but didn't read them. Sam arrives and asks to talk to House in private. She admits that she made most of the mistakes in their relationship, but that was a decade ago. Sam doesn't want him to like her, but they both care about Wilson. She just wants a chance to see if she and Wilson can build a new relationship. Once she leaves, House looks at William's sword and gets an idea.
House explains to the team that he has hepitus lesions indicating endocarditis, which confirm William was abusing steroids. The hemlock made the steroid damage worse. As they begin treatment, Thirteen asks why he would cheat. William explains that cheating to win a fight wasn't a game, but Shannon's feelings were real. Thirteen tells him to be honest with Shannon, but William insists that Miles can give her everything that he can't. Thirteen figures that he's an idiot.
In his office, House considers Sam's file for a few moments, and then throws it away and reaches for his Ibuprofen.
