Abbey is giving birth as her daughter Justine arrives in the delivery room. She delivers her baby, Kayla, and they realize that it's hypoxic. They start treating it and Abbey demands answers.

House tells the team to review the case, and Cuddy comes in to tell House that there's no one who can babysit for Rachel, and he's the only one left. She insists that Rachel won't be a problem, and he offers to do it in return for sex. Cuddy refuses to bargain, and notes that House still hasn't picked a female doctor to replace Thirteen. He agrees to babysit and then returns to the differential. House suggests liver failure and tells them to fix it to relieve her breathing problems. As they go to treat, House has Foreman stay behind and apologizes for choosing Chase to pick a woman. He informs House that he's already interviewed a candidate.

House is at Cuddy's house watching cartoons and listening to Rachel on the monitor. When Cuddy calls, he fakes checking in on the girl and then goes to raid the refrigerator. Rachel comes out and wants juice, and House sends her back to bed. She runs off and House calls Wilson.

The team tends to the baby and explains to Abbey what they're doing, and assure her that if they're successful, she can take her home. Abbey asks to hold her daughter but they say the baby has too many tubes. However, they find dilated bioducts and call House, who is trying to keep quiet. When House learns that Foreman has hired Dr. Christina Frazier, House immediately fires her as a lesson to Foreman, and then tells Taub to pick someone. He tells them to open up the baby and operate, and then hangs up. Wilson arrives and realizes why House wants him: so he can leave. Wilson tries to stop him but when House goes, Wilson goes out with him. House figures that Wilson will cave, but he threatens to call a babysitting service and then tell Cuddy. House agrees to stay if Wilson will, and they go inside to discover that Rachel has eaten House's Chinese food and at least one quarter from the change for the food.

The team operates on the bioducts with the aid of Dr. Cheng, but the BP drops and they have to pull out. They realize it's more than the liver, and must be related to the heart. Taub suggests that they might hire Cheng, but Foreman and Chase refuse to get involved because they figure House will hate anyone that any of them hire. Taub figures that they can get House to think it's his idea.

House uses a refrigerator magnet to see if Rachel has eaten any other coins, and Wilson notes that they're short a dime. Cuddy comes in and Wilson slips out the window so Cuddy won't find him there, and tells House to monitor Rachel for at least 24 hours until the dime passes. House then lies to Cuddy and tells her that Rachel is fine, and offers to stay the night. She accepts his offer and they go to the bedroom.

Taub comes in to check on the baby, and notes that Abbey's daughter Justine has gone to the cafeteria. Abbey admits that at least Justine is there. Taub notices that Kayla is looking healthier, and has Abbey hold her. Justine comes in and sees them together. Taub goes to give Cheng an update, and suggests that she join the diagnostic team. Cheng readily agrees to an interview, and Taub says that she may not need to interview.

Cuddy sleeps while House jumps out of the bed the first time that Rachel makes a noise. He checks her diaper with a flashlight, but no sign of a dime. Cuddy comes in and wonders what he's doing, and House claims that he's new to it.

In the morning, Cuddy prepares to go to work and House claims that he has the case under control and is going to sleep in. However, the maid, Marina, arrives. Foreman calls and tells him that they didn't do the surgery. Taub mentions Dr. Cheng spotting a vascular malformation, but House insists that the baby's ALTs were too high to indicate a vascular malformation. Taub then blames it on Foreman, but House isn't fooled. When Foreman notes that they treated for a vascular malformation, House says that it won't work and to call him when the baby crashes.

Justine talks to Abbey and notes that she's more caring with her new baby than she ever was with Justine. Abbey wonders if Justine considers her a failure as a mother, and Justine notes that she never realized that there was an alternative. They realize that the baby is bleeding again and calls a nurse.

House and Wilson sneak the sleeping Rachel into the hospital and manage to avoid Cuddy. They take her to a clinic room and try to use a sonogram to find the dime. Taub calls to tell House that the baby crashed, but House realizes he's in a no-cellphone area. He calls Taub on the house phone and tells him the liver is still failing, but Wilson calls him over to show him the dime. House figures they need to do an x-ray. Taub comes over and House goes out and tries to lie, and Taub wants to know what they do now. House wonders why the baby got better, and Taub says that they gave her steroids, which could have helped with idiopathic hepatic fibrosis. However, that doesn't account for the bioducts. Wilson calls him back in to reveal he found the dime, and House orders laxative. He then gets an idea and tells Taub to have Abbey give a direct blood transfusion. It relieves Kayla's symptoms, but House admits he has no idea why the transfusion is helping.

House meets with the team to figure out why the blood made a difference. Taub calls Cheng over and she does an adequate diagnosis. House suggests a deficiency of enzymes or vitamins is so likely, and tells them to try someone else's blood. Taub volunteers, and House calls Cheng along with him to test her... on whether a baby is safe after eating a dime. She tells him that it needs to be removed with in six hours, or he has to go in with a scope. When she wonders if she passed, he tells her to talk to Taub.

Wilson has Rachel in the men's room and House finds him. He tells Wilson to wait six hours and use the scope, but Wilson warns that it's dangerous. House sees Wilson's radiation dosimeter and gets an idea. He hides in the shower and treats it with low-level radiation until it registers positive, and then scrapes off Wilson's code.

Taub tells Abbey that they need to identify the infection. She explains that she had the baby so she could be a special mom.

Cuddy comes to see House in his office, and she says that they have a radiation leak somewhere because they found a radiation. She has to work late and reluctantly asks House to babysit again. He protests until she promises to make it his worthwhile, and then he gives in.

Taub and Foreman take a blood sample from the infected area, and they realize that they were wrong. It's a melanoma, and they suggest that Abbey gave her baby cancer. Taub finds a mole under her index finger and confirms that Abbey had skin cancer. House isn't impressed, and figures that it's metastatic which means it's late-stage. He wonders how she can be healthy at that stage, and figures that Abbey has antibodies and they're fighting something else involving auto-immune and the melanoma was caught in the crossfire. He tells them to test the blood. Taub takes blood samples from Abbey, who notes that she took all the precautions but missed her own cancer. Once he's done, he tells Abbey and Justine that they'll have results soon. Once he goes, Justine apologizes for what she said earlier, and admits that she was jealous. Abbey admits she doesn't deserve such a good daughter.

Taub and Cheng check the blood, and she tells him that House said it was Taub's decision. He figures that it's a setup and she admits that she agrees that House has an ulterior motive. They confirm that Abbey doesn't have auto-immune.

Wilson is at Cuddy's house checking Rachel's diaper, and tells House that there's no sign of the dime. He says it's time for the next step and grabs a scope

As they do a MRI on Abbey, Foreman tells Taub to make the decision and hire Cheng, and warns that he's overthinking it. They find AOT cell lung cancer and suspect that the cancer is fighting the other cancer. They inform Abbey that she has skin and lung cancer, and the lung cancer is treating the skin cancer. The lung cancer is treatable because they found it early, and Kayla will need chemo once they remove the tumor from Abbey. However, Abbey wants to keep the lung cancer in long enough for Kayla to heal, eight to nine days, but Foreman and Taub warn that it could cause her permanent damage. Justine warns against it, but Abbey says that she wasn't there for her and she turned out great. The older daughter refuses to let her mother martyr herself, but Abbey insists on waiting for her surgery.

House arrives at Cuddy's house with Chinese food and a scope, and he realizes they forgot to put in the extra pancakes. When they realize that House ordered the extra pancakes the other night, the bill was different, and there was no extra dime. They figure the dime was just a pocket of air, and House points out that Wilson reacted irrationally.

Justine brings food for Abbey and finds her dead. Taub informs Cheng that she died of a pulmonary embolism, which wouldn't have happened if they'd remove the lung tumors. They harvested enough blood to keep Kayla alive. Taub offers her the job, but she points out that he's been acting like a scared kid, and she hates kids. She turns down the offer and leaves.

Foreman comes in and finds Justine vowing to make sure the baby will know what Abbey did for her.

Cuddy and House are trying to sleep when Rachel makes a noise on the monitor. He tells her that it's Cuddy's turn and she gets up to change her daughter's diaper... and finds a dime. When Cuddy wonders where it came from, House listens as Rachel says one word: "House."
