A waitress, Nadia, is cleaning up and comments and describes a couple who were there a year ago, reciting details with perfect memory. However, the husband realizes that the woman was there with another man. As Nadia goes to get some water for them, she collapses and complains that her legs are paralyzed.
House brings Nadia’s case to the team and is happy to tell them that Nadia has absolute recall. He figures the symptoms are related, but Foreman disagrees and figures it’s a toxin. House sends Foreman and Taub to check Nadia’s home, and then gives them the second case: Taub, who just failed his pathology recertification. Taub claims that he was recovering from the flu and isn’t an issue, and Cuddy wants to make sure he passes. House tells him to pick a tutor, and Taub chooses Foreman.
Next, House goes to see Wilson and offers to take him out to pick up women. Wilson’s patient tells him to give her regards to Sarah. Once they’re alone, Wilson says that he’s busy and can’t go with House. House figures it has to do with Sarah, but Wilson claims his patient is senile. When House asks Cuddy the same question, she gives him the same answer.
As they examine Nadia’s house, Foreman quizzes Taub, who explains that he choose Foreman in the hope that he wouldn’t care. Foreman finds an empty box of laxatives and they figure she has magnesium poison. They call House, who is breaking into Wilson’s car. He finds a baggie of something marijuana-like in the trunk, as well as insulin syringes, and dismisses Foreman’s theory since Nadia’s BP is normal.
Chase and Masters are testing Nadia, and Masters asks why she’s content as a waitress. Nadia explains that just because she can remember it doesn’t mean she understands it, and she loves her job. House comes in and asks her if she’s clumsy, and then has her recount how many trips and falls that she had in 2008. Nadia tells him that she had 20 in the last year, a big increase over the last two years. That and the constipation suggest Parkinson’s, and House tells them to test and treat.
Wilson comes home and discovers that House has broken in. House shows him the baggie, which contains catnip. Wilson admits that he has a cat, and House points out that he’s adopted a diabetic cat named Sarah. He figures that Wilson has issues, and Wilson says that his neighbor died so is taking care of her cat. House figures it would be easier to drown it in the nearby river.
As Masters treats Nadia, she asks if Nadia has any tricks to remembering. Nadia wonders if her perfect memory will go away if the cure the Parkinson’s. Her older sister Elena arrives with daisies, and Nadia points out that as a child she was stung by bees from daisies. She has an arrhythmia seizure and the team stabilizes her. Back in differential, House is more interested in if Masters would date a man with a cat. They figure that Elena’s presence put stress on Nadia’s heart, and House tells Masters to use Elena to provoke another seizure.
House goes to see Cuddy and explains that after Wilson’s second divorce, he became obsessed with a cat until House “accidentally” disposed of it. Cuddy tells him to leave Wilson alone.
Masters and Chase monitor Nadia, who doesn’t respond to Elena’s presence. Chase sends Masters in to provoke a fight and she reluctantly agrees. The sisters argue until Nadia’s heart acts up again, and Nadia angrily orders Elena out.
Taub returns to his hotel room and finds Foreman waiting for him with a practice test. However, Taub isn’t interested, insisting that he has three days and he’d rather watch a movie. Disgusted, Foreman tells him to do what he wants, and spoils the movie for Taub as he goes.
Masters tells Elena that they’re treating Nadia, and Elena complains that her sister never lets anything go. As she starts to storm off, Masters admits that she provoked the fight. Elena warns her that she can’t play with people, and she’s just as bad as Nadia. Chase calls Masters in to smell Nadia’s brother, which has the odor of ammonia, and realize that her kidneys are shutting down.
The team goes over the symptoms and House dumps the insurance forms on Foreman since he has backed out of tutoring Taub. Taub claims that House must care about him, but House warns him that he can’t disgust him into letting the matter drop.
At home, Wilson is sneezing as he holds the cat. However, he goes to see House and accuses him of planting something to make him think he’s allergic to the cat. House denies it and invites him to prove he’s not overreacting by going out with him to a bar that night. Wilson reluctantly gives in.
As Chase prepares Nadia for dialysis, she insists that her memories and thus her truth never change, so her sister has done more bad things to her than good. Nadia starts to have breathing problems.
Foreman takes Taub to his home to study, and warns him to stay in the dining room and avoid touching anything. He insists that it’s now about his future as well as Taub’s.
House comes to see Nadia and discovers that she has hives. He realizes that she does have an allergy... to the dialysis that will save her. Masters wonders they can do, and Chase points out that Elena won’t donate a kidney. He explains that Nadia tallies everything and House figures that she’s right, and that she doesn’t let emotions cloud her judgment. He tells Masters to exploit Elena’s guilt, pointing out that she’s the one who messed things up.
Masters talks to Elena and stumbles through an explanation of her feelings. Elena cuts to the chase and says that she’ll donate a kidney.
Taub and Foreman play video games, and Foreman insists that he takes a practice test on the next break. After hesitating, Taub admits that he choked on the test, because he got a perfect test originally. He worried that the new test would reflect badly on him, now that he’s lost his wife and home. Foreman admits that he sometimes had trouble sleeping because of worry over a diagnosis, but assures Taub that things work out.
Chase tells Nadia that Elena is donating a kidney, and thought she should know.
After the operation, Chase checks on Nadia, who goes into convulsions. The steroids aren’t working, and they wonder what to do next. House asks them to check and see if Nadia thanks Elena for donating a kidney. He figures the kidney donation should trump any other harms, and wants to know if Nadia is perfect, or sucks like the rest of them. Chase suggests clotting and House tells them to threat.
Foreman reluctantly admits that Taub only got a 54 on the test, and Taub wants to steal a copy. He insists that he can’t handle another failure, but Foreman tells him that they need him on the team and he’s a good doctor. They’re going to keep taking the test until he passes. Taub doesn’t believe it and buys the test answers, while Foreman stands watch. He tells Taub that they can make sure he knows what he has to once he gets past the test.
Nadia asks Chase to take her to see her sister.
House and Wilson go to bar and objectify women. Wilson recognizes one woman as his barista, approaches her, and she quickly agrees to go home with him. House orders champagne for everyone... and then leaves before he gets the bill.
Nadia goes to see Elena and thanks her for saving her life, and talks about how they went to the lake in the summer. As they talk, Nadia remembers everything that has happened between them and breaks into tears. She tells Elena that she can’t do this and has Chase take her out. Outside, she asks Chase to stop pushing her for a minute. He figures that she has stayed in a diner, alone, because she can’t forgive anyone and she focuses on the bad memories. Nadia claims that she doesn’t have a choice, and Chase notices that she’s suffering from choreia, meaning their current diagnosis of a clotting disorder is wrong.
House is at Cuddy’s place considering who to set Wilson up with if the barista doesn’t work out. Cuddy figures that he’s feeling guilty because he has someone and Wilson doesn’t, and she assures him that his happiness doesn’t affect Wilson’s. The team calls to tell House what they’ve discovered, and Foreman comments that there were dozens of jigsaw puzzles at Nadia’s apartment. He orders them to do a blood smear and goes to see Nadia. He asks her about the jigsaw puzzles, and she admits that she can’t leave them unfinished. House then adjusts her water glass, and she keeps adjusting it back. He explains that she has OCD, and she hoards memories as a symptom. She has McLeod Syndrome, a rare genetic disorder that affects the blood and causes OCD. It can’t be cured, but the symptoms can be managed with medication and she’ll live potentially 20 years. House tells her that if it’s any solace, everyone dies alone.
Foreman comes home and Taub admits that he did fine since he had the answers. He gives Foreman his big-screen TV and thanks him for his help. After a moment’s hesitation, Foreman invites Taub to move in with him and Taub accepts.
After Nadia receives treatment, Chase talks to her and tells her that he doesn’t believe what House said. He has SSRIs that she can take for her OCD. However, Nadia insists that her memory is the only thing that made her special, and Chase says that if she wants it, she’ll have to be alone. He puts the pills on the table and leaves.
Wilson comes home and finds a mouse. He grabs a pan and chases after it, and House comes in with the cat. He explains that the mouse is for Sarah, and then tosses the ragweed that he planted out the window. After waiting a moment to see if the cat leaps out the window, House asks for details about his date. Wilson finally admits that he bailed, and he needs more time. House smiles and says that he’ll give his friend ten days.
Nadia considers the pills and finally takes them.
