Business executive Patty Mishner with a women’s right group is berating her assistant Lou and sneezes while discussing their business schedule. She arrives at a meeting of men to talk to them about their problems with women, while Lou starts to hallucinate being covered with ants and screams in terror.
House is playing video games in Coma Guy’s room when Cuddy comes in to inform him that Wilson is back after a two months’ absence. She wants House to talk to Wilson but he’d rather take Lou’s case. He goes into differential with the team and Thirteen leaps to Lou’s defense, saying it isn’t her job that made her ill. House disagrees, then tells everyone that she has Huntington’s. He tells Thirteen to treat Lou for vitamin deficiency brought on by job stress, and the rest of the staff ask her about it. She says she hasn’t been diagnosed with Huntington’s, and accuses House of deflecting his issues with Wilson onto her.
Thirteen treats Lou with B-12 injections but Lou starts bleeding anally. Meanwhile, House confronts Wilson, who informs him that he plans on resigning. House tries to talk him out of it, warning that it’s standard grief reaction, but Wilson isn’t buying it. Thirteen arrives to inform House about Lou’s condition and he tells her and the others to deal with it. Taub is fine with that but Thirteen is angry that House is taking the time to deal with personal issues. Taub doesn’t discover any source of bleeding but Kutner comes up with a potential diagnosis based on the admin tests: she’s pregnant. As they prepare for an ultrasound, Lou says it’s been two or three months since she’s had sex. The ultrasound proves negative despite the pregnancy test.
Back in differential, House is still distracted by Wilson’s issues and tells them to continue with the diagnosis while he confronts Wilson. Wilson refuses to discuss the matter and the rest of the staff comes in. House informs them Wilson is leaving and then insists that Lou is pregnant. He goes to her room and does another ultrasound and reveals that the fetus is in her intestines and the pressure from it is causing the symptoms. House tells them to remove it and she’ll be cured. He leaves but Thirteen follows him and insists that the fetus might be able to be saved. House notes that Thirteen is taking Lou’s case personally because of her own medical condition.
Thirteen goes to Lou, who immediately agrees to the removal of the fetus. Meanwhile, House talks to Cameron about Wilson, but she insists it’s up to Wilson and she’s not going to interfere. As Lou undergoes surgery, House goes to Cuddy and suggests she block Wilson’s transfer. Cuddy refuses and tells House to talk to Wilson and apologize to him. When he refuses, Cuddy wonders at House’s lack of guilt and tells him to say he feels bad anyway. House refuses, insisting it’d be meaningless.
Lou is suffering from neurological problems and they conclude that the pregnancy is unrelated. They try to page House, but he’s in Wilson’s office and ignores it. He asks if saying he was sorry would change anything, but Wilson is skeptical. When Wilson still refuses to stay, House insists he won’t do any work until Wilson agrees. The rest of the team is trying to resuscitate Lou as House leaves the hospital.
Cuddy meets with the team, who inform her that House isn’t taking his calls. Cuddy assures them she has complete confidence in them, and Taub suggests they just send someone to service House. Thirteen insists they don’t need House and suspects Lou has MS. The others are skeptical but Foreman okays the treatment. Meanwhile, Cuddy goes to see House and insists that he apologize, but he refuses.
Thirteen visits Lou, who wonders where House is. Thirteen lies and says House is still part of the team, and then notices that Lou is running a fever and they’re wrong about MS.
House goes to Cuddy’s office to confront her over the fact that she had his cable shut off. She’s taken the precaution of taking the remote from the doctor’s lounge. Wilson arrives and Cuddy informs him that she isn’t helping Wilson transfer until they sit down and undergo “couple counseling.”
Thirteen goes over a recording of the surgery, suspecting she became infected during the operation. Kutner notices a ganglioma on the tape, and Foreman orders Lou opened up again so they can take a biopsy.
Cuddy insists that House tell Wilson what she’s feeling. House and Wilson team up against her and Cuddy ends up trying to get through to Wilson, telling him Amber wouldn’t want him to walk away. Wilson notes that nobody even liked Amber and storms out.
Chase is skeptical about a second surgery being necessary and insists the bump isn’t a ganglioma. He warns Lou can’t survive a second round of anesthesia, but Thirteen notes that Chase would do it for House. He refuses, but Kutner suggests they run a lighted probe in through the rectum and push the ganglioma the surface, then cut it out for analysis. Thirteen informs Lou about the procedure and she insists on seeing House. However, Lou reveals that Patty fired her but she feels fine about it. Thirteen gets mad at Lou, noting she’s letting herself be treated as a flunkie and they can have anything. Lou notes that they can aspire to anything, but they can’t have everything and she’d rather be close to someone in authority then taking such a position herself.
The team finds the ganglioma and push it up to the skin for removal. Meanwhile, Cameron talks to Wilson and notes that she thinks House is right. She warns that Wilson thinks he’s making a rational choice, but based on her own experience she warns that in six months it’ll hit him again. Wilson confesses that everything in the hospital reminds him of Amber, but Cameron says she still has the same remembrances about her dead husband.
The team analyze the lump and determine it’s amydosis and they need to cure the cause. Taub warns that they don’t have an answer… and they don’t have House. Foreman goes to Wilson to make the call on the possible source. Wilson suspects lymphatic cancer and Foreman agrees to treat for it, then advises Wilson that he needs to leave if it’ll make his life even a little bit easier.
Thirteen stops in to see Lou, who says the treatment is making her feel better. Lou thanks Thirteen, who responds by admitting she has Huntington’s. Lou believes that Thirteen wants to make a difference and admits that she’s different for having met Thirteen. Inspired, she’s applying for a job running a finance division at a foundation.
House manages to find a remote but Cuddy asks him what House really thinks of Wilson. She notes that House didn’t insult Wilson, and he’s afraid that he might find out what is really bothering Wilson. Distracted, House ignores her and goes to see Lou. He notes that she looks more like her age of 37 than earlier when she looked 27 due to her youthful skin. He orders them to stop the treatment and takes a blood sample from a bruise on her leg. He determines she has “pretty” leprosy that made her skin look smoother. The chemo treatment made her feel a little better when it wiped out the bacteria, but would have eventually wiped out her immune system and killed her. House leaves for his soap opera and Thirteen is angry that House was right.
Later, Thirteen explains the diagnosis to Lou, who tells her she’ll be working with Patty after all. Thirteen wonders if Patty will change, but Lou admits she doesn’t want her to change. Afterward, House goes to see Thirteen, who admits she thinks Lou is pathetic. House notes that almost dying changes nothing: dying changes everything. Realizing what he’s said, he goes to see Wilson and apologizes, saying he feels bad. Wilson says he doesn’t blame House, but they’re not okay. He explains that he’s not leaving because of Amber, but because he’s enabled House for years and he’s finally sick of it and he’s putting himself first. Wilson tells him they’re not friends any more… and maybe never were.
