Big Baby - Recap

Kids are playing in a Special Ed class when one of the boys notices a new assistant. He asks the teacher, Sarah, why she isn’t married and she admits she hasn’t found the person she loves yet. He asks if they get married and Sarah puts her off. Johnny spills glitter on another girl’s picture and Sarah tries to comfort her, but starts spitting up blood before she collapses.

House arrives in the cafeteria and steals some food. Cameron confronts him about it and then brings him a case. She informs him that she’s been put in charge of him while Cuddy spends more time with her baby. House tries to undermine her but she’s unimpressed and gives him the case. In differential, House notices that Foreman is smirking about the fact that Cameron is now in charge. House notices it won’t last, and comments that Foreman and Thirteen are disagreeing to cover up their romantic interest. He tells them to run a bleeding-time test to check Sarah’s blood.

Kutner and Thirteen test Sarah, who says she goes to her “happy place” and is incredibly positive about her work with her students. There’s a lack of clotting but she tells them she’s not worried about what the tests might show.

Wilson visits Cuddy and she starts crying. She admits she isn’t feeling anything at all for Rachel but insists she’s doing everything she’s supposed to do.

Thirteen reports that Sarah’s platelets are chewed up, but House is more interested in Thirteen and Foreman disagreeing and then not disagreeing. House suspects ITP orders massive radiation treatment but the staff objects, and House says it’s somebody’s job to stop him. He goes to Cameron who tells him to go ahead and she’ll approve whatever he says. He asks for oral sex and Cameron ignores him. Stymied, House goes to his team to find a way to not use the radiation treatment. Thirteen suggests they book the equipment but don’t do the radiation.

Foreman talks to Chase and reveals that he accidentally learned that Thirteen is on the placebo. Foreman wants to switch her to put her on the real drug, and Chase notes that it’d be stupid to do so.

As Thirteen and Taub monitor Sarah as she undergoes the (non-existent) treatment, she says she has to urinate. They let her go and she collapses on the floor, They’re forced to resuscitate and they realize it’s not ITP.

They check his heart but it shows clean. House wonders why she had to urinate and insists people don’t die from peeing. Thirteen suggests that the cold temperature may have caused cold agglutinins that put strain on her heart due to lower temperatures. House tells them to give Sarah an ice bath to activate them.

Cuddy confronts Cameron about approving House’s procedure, and Cameron explains that House was bluffing. Cuddy warns her not to play House’s games but Cameron says she’ll do what she needs to. House comes in and waits until Cuddy leaves, and when she stays he starts speculating that Cuddy is having second thoughts. Cuddy says she was having a bad day and isn’t thrilled that Wilson told House. House suggests that Cuddy give Rachel back rather than leave the girl with a mother who doesn’t give a crap. Cuddy stalks out and Cameron gives House permission to do the ice bath, as soon as he does a blood test and chills it to confirm his diagnosis.

Kutner goes to Sarah’s room and sees Johnny and his mother visiting. Johnny’s mother explains that Johnny was autistic and Sarah helped bring him out of it. Kutner agrees to let them stay.

As Foreman and Thirteen test the blood, Thirteen says she’s feeling better and Foreman warns against her getting too optimistic. Thirteen refuses to think negatively. They finish testing the blood and discover the blood has clumped. Foreman goes to see Cameron, who refuses to talk to him.

On Cameron’s orders, House watches as Kutner and Thirteen put Sarah in the ice bath. Kutner has her talk about how she got Johnny to overcome his autism, by becoming a part of his world. She also talks about how she walked into the wrong classroom by misreading the room number. House is less than thrilled at her upbeat nature. Her heart rate remains normal and House concludes she has brain damage, based on the room number mix-up and her inability to judge when she has to urinate. Foreman disagrees, noting House is just trying to find a reason to justify she’s a nice person. Next, House suggests poor blood flow due to MS plaques in the brain and the lungs will be next. He suggests a brain biopsy and Kutner disagrees, suggesting they run an ERCP on the pancreas. They go to Cameron who says they have to do more MRI tests first. House refuses, saying she’s wasting time by hedging her bets without giving into him. She tells him to do the MRI.

Cuddy brings Rachel to Wilson’s office and tells him she doesn’t want to go home. She admits she wonders if House was right, and she doesn’t know if she wants to sacrifice for the baby.

The MRIs prove negative and House okays the ERCP Foreman talks to him in private and asks House about the ethical aspects, and House notes that he must have already talked to everyone else and hasn’t got the answer he wants. House asks if Foreman loves Thirteen, noting that people in love do stupid things.

As Kutner, Thirteen, and Taub run the ERCP test, Sarah’s lungs start to shut down just as House predicted. House suggests a virus that the MRI wouldn’t pick up and proposes a neural conduction test on her exposed brain. Kutner wants a second test and goes ahead anyway, saying House should get permission. He goes to Cameron who already knows what he wants and he says he suspects equine encephalitis from a mosquito bite set it off. She wants a confirmed test before he cuts open Sarah’s skull. House warns that her playing it cautiously could get Sarah killed.

Thirteen and Foreman check out the classroom and Thirteen talks about wanting children now that she thinks she’s feeling better.

Wilson visits Cuddy at home and gives her a picture of Rachel, aged by computer to what she’ll look like as a teenager. Cuddy is less than impressed and Wilson admits it was the picture that came with the frame.

House reports to Cameron that they found three dead fruit flies and a kid with the common cold. However, Kutner’s pancreas test has proven negative. Cameron still won’t approve the biopsy and she asks if she should say “yes” just because he’s House. He says she should and she gives in. Kutner insists the procedure is insane and warns that House is skipping steps to test Cameron. He suggests they should check her spleen instead but House refuses. He and Cameron begin to test Sarah’s brain while Kutner calls Cuddy. He puts Cuddy on the line and she tells House to step away while the baby cries in the background. Sarah’s blood pressure starts to drop and she says that the baby’s crying is annoying. Sarah’s nerve conduction increases. House wonders why the baby is annoying Sarah, while Cuddy yells at Rachel trying to figure what she wants. The baby calms down as Cuddy starts crying and asks what she wants.

As they wait for the anesthesia to clear from Sarah’s body so they can check her spleen, House considers the case and notes that Sarah’s blood pressure was low at the time.

Cuddy brings Rachel to see House, and he notes that her talking to her helped her connect with Rachel. Rachel spits up on House and he thinks about her stomach sphincter.

House visits Sarah and insists that it’s her brain, not her spleen. He checks her heart and discovers she has a patent ductus arteriosis. The valve didn’t close when she was born as is typically the case. When she gets stressed, the ductus opens and the blood leaks into her brain, lungs, and her nose. The blood in the brain flows away from the left side of her brain, reducing her stress. When her blood pressure dropped, she felt annoyance. They can fix the heart and alter her mood.

Cameron visits Cuddy, who says that Cameron was right. Cameron informs her that she’s quitting. She admits she’ll always say yes to House, and anyone else would say no. Cuddy realizes that means she’s stuck with the job.

Johnny comes to see Johnny, who spills water on her. Sarah is fine with him doing so. House observes her still being cheery and upbeat.

Foreman gives Thirteen the medication instead of the placebo.

Cuddy leaves Rachel with a babysitter and goes back to work. Rachel starts to cry… and Cuddy keeps going.
