A woman, Victoria, approaches an old house where a rave is occurring. She offers to pay the $20, but the doorman figures she doesn't have it. When she gives a fake story about a boyfriend named James, he lets her in. Once inside, Victoria calls out for James. A girl jokingly offers to help Victoria find James, and then hits on her. Her girlfriend shoves Victoria into the wall, and then the police arrive to break up the rave. When Victoria tries to interfere, she collapses and the cops take her to the hospital.

Wilson asks Foreman to examine Victoria, who has been admitted for a supposed drug overdose. However, her tox screen is clean. Foreman isn't impressed, figuring that she's homeless and trying to run a scam, but Wilson notes that Victoria has lesions on her arm and a twitch of her right wrist. Foreman humors Wilson and examines her, but says that she's faking it. However, Victoria has a seizure and they confirm that it's real and she has low blood sugar. Afterward, Foreman figures that she's diabetic and overdosed on her own medication to get a place to stay and some free meals. He tries to check her bag, but it smells too badly for him to examine. Foreman dismisses the twitch and tells Wilson to watch her for a few hours and then discharge her.



Wilson goes to House and insists that there is something wrong with Victoria, and Foreman isn't being objective. House is intrigued less by the case than by Wilson's persistence and agrees to take a look at the patient. He takes it to the team for differential, much to Foreman's irritation. He tells them to check a MRI for a brain tumor and then discharge her when it proves clear. House tosses out her bag and says that the big question is who she is, and Foreman wonders why he's so interested for no reason. House ignores him and says that they need a medical history. Her bag contains an insulin needle, but also a sweater soaked in vomit. House tastes it and says that it's salty, indicating an electrolyte imbalance. He ignores Foreman's suggest for a MRI and orders 24-hour treatment for the electrolyte imbalance. Wilson thanks him and leaves. As the others go, Foreman asks House why he's so fascinated, and House notes that he's interested in why Foreman doesn't to take it.

As Chase administers treatment, Victoria is doing cartoon sketches of someone looking for James, standing outside a pawn shop. She suddenly has a seizure and goes berserk. When Foreman tries to restrain her, she bites him on the wrist. Chase confirms that she's negative for hepatitis and AIDS, but Foreman says that he's giving her a MRI and then getting her out. He switches another patient, Mrs. Whitney, for Victoria so that they can get her in and out. Cuddy realizes that House has made the switch, stops the scan, and confronts House and Foreman. She warns them that Victoria has a surgical pin in her arm, and the MRI magnet would have ripped it out. House suggests they remove the pin, and bulldozes Foreman into agreeing she has a brain problem so she needs a MRI scan, and then going out to find out more about her.



Foreman finds the pawn shop from Victoria's drawing, and a homeless man, Walter, offers to direct him to Victoria's stuff in return for Foreman's coat. Foreman agrees and finds bats in the box where she sleeps. He also finds a folder and takes it back to the team. They've determined that the MRI shows no sign of a tumor, but Wilson insists that something is going on. Foreman shows them the comics that she drew, and House examines them and concludes that she's from Philadelphia, and they indicate a car accident two years ago. The team has no idea how he's doing it, until he tells them that he traced the surgical pin he conned Cuddy into removing. He's confirmed her name is Victoria Madsen. Foreman is more interested in her blood work report, which indicates she's allergic to the electrolyte treatment. They rush to her room and discover that she's having a respiratory arrest. They struggle to revive her while Wilson looks on.

Back in differential, the team confirms that Victoria has visited three hospitals in the last two years, but no records of a home address. at one hospital, she was treated for frost bite, and another for depression. Chase finds a billing record for two ultrasounds, ten months apart, and Victoria blew off the second. Wilson suggests abdominal pain, and the nurse made the second appointment without consulting Victoria. They were looking for ovarian cancer, and the appointment was with an oncologist. When Foreman says that cancer tested and wouldn't account for the twitch, House suggests neoplastic syndrome and orders an ultrasound of her ovaries.



Cuddy assigns House to a woman to deal with her kids, and he fakes a cold to leave. He goes to his office and reads Victoria's comic books, and suggests to Foreman that they might reveal something about her personal life. Foreman isn't interested, and House figures that he has a personal stake. When Foreman angrily stalks out rather than answer personal questions, House tries to get more from Wilson on his interest, but he insists he doesn't have a personal stake. Cuddy arrives with two students and orders him to teach personal histories. The two students, Julia and Chris, are eager to learn, and House sends them to interview the patient that Cuddy assigned to them.

As Chase and Cameron prepare the ultrasound, they speculate about House's interest. They confirm that she has cancer.

Wilson finds House hiding out in the clinic to avoid the students. House is going over Foreman's personal records to figure out why he hates homeless people, and then reveals he's going over Wilson's files as well. They get a page from Cameron and Chase about the cancer report, and Wilson confirms there's nothing can do for her. House suggests that it's a tuberculoma caused by TB. He admits that it's less likely than ovarian cancer, but orders treatment anyway since the alternative is doing nothing.



Victoria does a sketch of Foreman and asks what he's giving her. He explains what he's doing, but she realizes that she doesn't have a tuberculoma. Foreman apologizes for not believing her, and Victoria admits that she overdosed on insulin. He asks her who James is, and Victoria says that he's real. Foreman offers to help her find him, but before she can respond, Victoria complains of photosensitivity. He confirms that her temperature is skyrocketing, turns off the lights, and offers her water, but she spits it out and claims that it's poison.

Foreman goes to see House and tell her that it must be cancer, and Victoria is dying. Chase comes in and announces that the lab results confirm it's a tuberculoma. House points out that the treatment for the diagnosis is killing her. They confirm the lab results and figure that an infection is responsible. While they run more tests, they put her into an ice tub, but Victoria begs them not to. Foreman tries to reassure her, but she convulses in agony when she touches the icy water.



Julia and Chris report to House, who is busy reading Victoria's comics. They try to talk about their patient, who has a sprained wrist. However, the patient claims she injured it both in a horseback riding incident, and at the beach. House realizes what's wrong with her, gives them a book of illnesses, and tells them what she has starts with "C." House gets a page and meets with the team, who report that the tests are negative. The lumbar puncture reveals elevated white counts, which indicates meningitis. House orders treatment for it. When the team goes to treat her, they discover that she's left despite the fact that she was sedated. There are more comics on the walls, showing her looking for James, and he realizes that Veronica's returned to the streets.

Wilson and Foreman report to Cuddy, who warns them that their liable if she got out of the hospital with contagious meningitis. Foreman rushes out to save her, figuring he knows where she's going. Cuddy tells him to call the police and let them track Victoria down.

Afterward, Chris and Julia try to figure out what their patient Jodi has, and notice that he's wearing a bird pin. He asks Jodi about her wrist, and she says that she was riding a Ferris wheel when a seagull flew at her. They students realize that she's lying, and House explains that she has no memory of what happened. Outside, he explains that Jodi has Korsikoff's Syndrome, and her brain fills in gaps with visual stimuli. House notes that Chris had a beach scene on her notebook, and Julia had a horse logo on her shirt. When Julia notes that Korsikoff's doesn't start with a C, House ask them if they're sure he said it or not. When they note that Jodi is crying, House goes in and Jodi has no memory of what just happened.



The EMTs bring Victoria in and inform Foreman that she was passed out at a state park, and is suffering from arrhythmia. After she's stabilized, the team runs differential since it's clear it's not meningitis. House confirms it is and tells them to treat, and then goes to talk to Gilmar, the police officer who found her. The officer sticks to his story that he found her laying on the grass, but House realizes that Gilmar used a taser on her. Gilmar still sticks to his story, and House bribes him for the truth. Afterward, he tells Foreman that they can account for the accelerated heart rate. However, Gilmar told him that he hit Veronica in the thigh and she ignored it. House sticks her with a needle and they realize she's lost all feeling in her thigh, but not her toes on the same leg. He takes a swab and then tests Foreman where Veronica bit her. He can't feel the needle. House lays out all the symptoms, and they realize that Veronica has rabies. The tests prove positive, and there's no treatment. She only has a day or two, and Foreman has to get his shots immediately.



Wilson administers the rabies shots and asks if Foreman wants him to talk to Veronica. He doesn't see the point since there's nothing they can do. Foreman insists on going out to find James so Veronica won't die alone. Wilson goes with him, and they follow the addresses in her comic books. They bust into a room in an old house, and find a lockbox with photos of Veronica and a man from a happier time. Wilson confirms that it's her husband, Paul, who Veronica drew as the villain in her comics. They finally find a photo of her child, James, and determine that the car crash she was in that broke her arm killed her husband and son. Veronica was driving at the time.



Back at the hospital, Foreman takes Veronica's hand. Delirious, she thinks that he's Paul, and Foreman tells her that he's there to forgive her. Sobbing, Veronica apologizes over and over again.

House tracks down Wilson, who is sitting on a doorstep in a poorer part of town. Wilson explains that he has two brothers but only told House about one, and the house he's sitting in front of was the last place that Wilson saw him, nine years ago. He doesn't know if he's even alive.
