Frank and Emma are on the trail of a young girl who has escaped from an asylum and kidnapped a young Sheriff deputy. Years before, the girl was found in her family's apartment living with their gruesomely murdered bodies. While she was in an asylum, the girl never spoke. Now, on the road with the captive deputy, she talks non-stop chattering somewhat nonsensically about every legendary paranoid fantasy and conspiracy. As Frank and Emma start to build a case that perhaps the girl is not responsible for her family's deaths, a close bond forms between the girl and the deputy.

Frank receives digitally-altered photos that show him as the victim of a fatal drowning incident. His childhood fears about water and death, as well as his unresolved issues over the death of Catherine, come flooding to the surface. While investigating who could have known so much about his past, he becomes involved in a series of incidents that lead many at the FBI to believe he is slowly losing his mind. After taking a leave of absence from the Bureau, withdrawing from his friends and colleagues, he falls into a deep pit of anguish and despair. Soon he comes to believe that an evil demon has targeted his psyche and is for some reason trying to push him past the point of no return.

When a dog digs up a newly severed female foot and ankle, Emma and Frank return to the small town in which she spent part of her nomadic childhood to investigate the crime. As figures from Emma's past keep popping up and the mystery slowly unfolds, the two FBI agents discover that this is not simply an isolated incident but one in a series of killings of college age women that have occurred over many years. Although using his "profiling" abilities Frank early on discovers who he believes to be the killer there is no real hard evidence linking the killer to the murders. Drawing upon his many years of experience dealing with the psychology of killers and using his unique talent to peer into an individual's psyche, Frank slowly "peels the onion" of the killer's mind.
