In the Ochoco Federal Prison, a Muslim group surrounded a young black man named Mo who was working in the prison kitchen and began to beat him in, an initiation ritual meant to welcome him into the gang. Falling to the floor under their blows, the young man curled into the fetal position, used his arms and legs to block the blows as best he could, taking punches in the face as well. Suddenly, the leader, Abdul Mombasa, motioned for the beaters to cease and told the young man that he was welcome to his new family and that he would be well taken care of financially as well, not wanting for anything ever again. They held the man up from the floor and gave him hugs and reassurances.


Mo was seen by the guards and taken to the prison's infirmary where he made a call to Sam, listed as next of kin, telling Sam that he had had no choice to not join the gang. Sam was at NCIS headquarters when the call came and after he hung up, he turned to his colleagues and said that it was an emergency and he had to go visit his friend. He was enlisted by Hetty to pose as a prison inmate and infirmary worker in order to find out what Abdul was planning since it was known to the NCIS that he was part of a very dangerous terrorist organization. Hetty ordered Eric to create prison identification documents made for Sam going back at least ten years and to create a fearsome reputation for him as well.


At a local cemetery, agent Callen walked to his sister's grave and kneeling down, placed flowers upon it. He saw an old toy lead soldier on the grave and picked it up, examining it closely. Who might have placed it there? He had set up a hidden camera on a nearby tree to catch any visitors to his sister's grave and two were found on the tape: one was a man wearing a hoodie that Callen had chased in a dream. Callen wanted to meet the man. As for the toy soldier, Callen remembered that he had been given one as a child by an older man. He showed Hetty the toy and she said that she had played with such as a child herself.


Sam told Hetty that Mo shouldn't be in the prison alone to which she replied that he was not. She said that Nate was inside posing as a prison psychologist and alert to Mo's concerns. Hetty gave resident electronics expert Eric a code to access and manipulate the prison cameras. Hetty told Sam that Abdul's brother was Sadat Habassah, head of a very dangerous international terrorist group headquartered in Yemen, and had access to bank accounts containing twenty million dollars. One of the terrorist cells was located in West Hollywood.


Kenzie and Deeks, posing as brother Edward and sister Jennifer, were sent to investigate a dance studio that posed as a front for the West Hollywood terrorist cel. Kenzie and Deeks talked to the dance instructor on the pretext of signing up for dance lessons when Kenzie asked to use the bathroom. Told that it was upstairs, she climbed to the second floor and found the office, taking pictures of documents there and logging into a computer, allowing Eric and his assistant to access the terrorist records. Eric found that there was a warehouse located in North Hollywood that might contain vans the group used. Hetty sent Kenzie and Deeks to check it out.


In the prison, prisoner Sam was taken to his cell and ran into the Muslim gang where he pretended to be Muslim, getting to know Abdul and his henchmen. Sam was sent to work in the prison infirmary where he secretly talked to Mo who was still in the infirmary, telling him that he was there to help and protect him.
Nate talked to Sam and afterward, called Hetty, telling her that Sam was too close to the gang, that he was in for big trouble. Hetty said not to worry.


At the North Hollywood warehouse complex, Kenzie and Deeks found their way into the warehouse, finding a silver van like the one they were looking for. Two men were there and began to fire on the pair who killed them. Opening the van's doors, the agents found a large amount of ammonium nitrate, part of a blasting cap and wires used for a bomb. They called Hetty and told her what they had found. It was decided that the bomb had not been intended for terrorism but for blowing a hole in the prison wall. Hetty decided that Abdul was planning a prison break and returning to Yemen, his brother and the terrorist group and that Sam would have to go with him in order to discover what the group was planning. Sam was informed of this. Nate told Abdul that he was being transferred and to come wth him. The two went down the hall where Abdul pulled Nate aside and stabbed him, leaving him in a puddle of blood on the floor. Abdul ran to the infirmary.


Chatter from Yemen was talking about a big prison break in Los Angeles which supported what Hetty was thinking. Sam got into a scuffle with two white prisoners and the Muslim gang helped him. When Abdul tried to recruit him for the prison break, Sam declined but later in the prison yard, decided to join them, telling Abdul that he could get him out today since Sam had purportedly been planning a prison break to escape himself.. Sam was in the infirmary working when Mo staggered in, blood pouring from his abdoman purportedly from a stab wound, but it was fake. Sam told a guard to call for an ambulance, Mo had to get to a hospital now! The guard did so and Sam and another man joined them in taking Mo to the dock where the ambulance would meet them. Abdul was hiding behind a door there and when the ambulance pulled up, he helped to knock out the two attendants and hide them He changed clothes and got into the ambulance with Sam, Mo and one other prisoner. Later, when the men were lodged in a nearby motel, Sam wearing a wire, Abdul walked up to Sam and told him he had been told that an NCIS agent was amongst them, but he didn't think it was Sam. Abdul said he was going home and left the motel. Sam walked into the motel room, finding finding prisoner Saeed dead of a stab wound and surrounded by blood. Was Mo still alive? Sam rushed into the bathroom, calling the young man's name, only to find Mo suspended by his arms, also dead. Sam held the boy in his arms and cried, then called Hetty and told her the news. She sympathized with Sam and told him to follow through with his mission, that that was the way it had to be. He would have to go all the way to Yemen and infiltrate the terrorist group to bring them down, that he could not protect everyone.


Back at NCIS headquarters, a tired Hetty placed a call to agent Callen, only getting his voicemail. She said, in the message, that she had not heard from him in forty eight hours and that it was time to check in.
