Hank is running on the beach when a woman catches and passes him from behind. Hank speeds up to pull ahead again only to fall to the sand in pain as he suffers a leg cramp. The woman stops to help him and it turns out to be a world class marathon runner names Jane Cameron. When she learns Hank is a doctor, she asks if he can help her just as Riza, her nanny appears with her daughter. Hank says he can work her into his schedule.
Evan awakens in bed and finds Paige sitting in a chair sketching him. He asks to see her work but she refuses to show him. Evan asks what painting she entered in the local Hamptons art festival and she says she missed the entry deadline.

Marisa is posing with her new baby Carlos for a professional photographer with Jill and Divya watching. The girls start asking Divya about her date with the doctor from Hamptons Heritage and she tells them it is not a date, just a thank you. Hank arrives and asks what they are laughing about and they say Carlos, maintaining Divya's secret about her job at the hospital.
There is a knock at the door and Hank says it is Boris. The girls tell him not to let Boris know Marisa is there as she wants to surprise him with the photos. Boris has brought his security chief and is alarmed when Hank tells him Marisa and the baby are not there but Marisa comes forward when he starts barking orders for his security team. Marisa shows him some proofs and returns to her baby. Boris starts sweating and appears ill so Hank asks him to come inside out of the heat and gives him a glass of water. Boris cannot hold the glass and it falls to the floor, breaking. Hank asks to examine Boris but Boris tells him not now.
Hank calls on Jane to discuss her medical concerns. She tells him she has been having problems since coming to the Hamptons and training in the summertime heat, getting weak and feverish. She thinks it may be heatstroke, related to the recent birth of her child. Hank is doubtful as the time line does not fit. Hank thinks the heat may play some part in her problems, but his blood test shows she is anemic. She is puzzled by this since she eats a regular diet including meat, so Hank says he will do a full blood test workup.

Evan shows up at Paige's house and when he realizes she is absent, he starts collecting some of her artwork to take with him. Before he can leave undetected, Paige's mother walks in. When Evan praises her daughter's artistic talent, she comments that none of Paige's other boyfriends ever noticed.
Hank and Divya arrive at Boris' home to give him a physical. Boris is convinced it is the start of his inherited disease but Hank tells him to not jump to conclusions. When he shows no fever, Hank recommends including Marisa in the consultation due to her medical expertise in the field. Boris demurs, requesting they await the results of the blood tests.

Evan takes Paige's work to the art festival and begs the lady in charge to include her work, even though she missed the deadline. The lady is reluctant so Evan plays the name card revealing Paige's well respected family name. When the lady looks through the samples Evan brought, she finds one that intrigues her and decides to allow it to be entered. It is a nude portrait of Evan that she drew on the back of another painting. Evan does not see the nude, thinking the landscape on the other side is what will be displayed.

Hank discovers that Boris is sending Marisa and his son away with the security team. He confronts Boris, thinking he is sending Marisa away because of the possible beginnings of his fatal illness until Boris confesses to him that he believes someone is after his son. He tells Hank that some of his cousins reacted badly to news of his possible heir on a recent visit to Germany. He finally told Marisa about the issue, and that Jill's accident was no accident so Marisa agreed to leave.
Boris also reveals he is starting to experience pain in his feet and legs, deep pain in the bone. Hank cannot believe it could be spreading so rapidly and wants Boris to go to a hospital for an MRI. Boris refuses and tells Hank to let Dieter know whatever he needs to accomplish his tests.
Hank visits Jane and lets her know her anemia is not diet related. He has a theory that she is suffering from a condition that sometimes happens to long distance runners and infantry soldiers. It is due to the repetitive pounding of the feet. Hank wants to do a stress test on a treadmill, but Jane declines saying she prefers to run outside where she experiences the problem anyway, and Hank can run with her.
At the art show, Evan and Paige are browsing the exhibits. She tells him she is glad she did not enter because she is not ready to show the world her work, it is too personal. Suddenly Paige realizes by Evan's attitude that he entered her work without her consent. Suddenly she sees the drawing of Evan, and tells him to turn around, where he gets quite a shock. To his horror, he notices a man studying the drawing closely, then finally walking away. Evan apologizes but Paige is not upset but thankful that Evan believes in her. Suddenly the lady in charge arrives and reveals that the drawing sold, surprising both Evan and Paige.

The results of Boris' MRI show no problems with his bones, and the anti-inflammatories that Hank gave him are helping with the pain. The results of the blood tests are not complete so Hank recommends he take it easy.
Hank returns to Jane's house and has her swallow a pill that contains a tiny thermometer that will broadcast her core temperature wirelessly to a hand held meter while they run. After a long run her core temperature shows 102 degrees, which is normal for distance runners. She picks up her young daughter and suddenly starts feeling dizzy. Hank grabs her daughter just as she collapses.

Hank gets her inside where she starts feeling better. His meter shows her core temperature spiked so he decides to redo her blood tests himself since the laboratory seems to be slow in processing the results. Hank feels her daughter's forehead and thinks she is also too warm, so he suggests they both take a cool bath while he processes her bloodwork.
The tests reveal she suffers from an inherited condition that makes it difficult for her to process iron properly. She was born with it but something recent triggered it into activity. Her daughter might also have it so Hank recommends they both see a hematologist for extensive testing to identify the trigger so they can avoid it and lead normal lives.
Hank, Jill and Divya arrive at the art show to see Paige's work. Evan tries to steer them away but they see the nude sketch of him and are quite surprised. Evan proudly tell them that the sketch sold to a famous gallery owner, Eric Kassabian who arrives with Paige. Eric praises Paige's work and she is quite overwhelmed with the flattery and walks away with him to the dismay of Evan.

Divya runs into Dr. Paul Van Dyke at the hospital and he invites her to a restaurant for a date. She has no opportunity to say no and Jill witnesses the encounter, amused at Divya's reaction.

Hank speaks with Jane on the phone where she tells him his diagnosis is confirmed. After hanging up he gets a text message with the results of Boris' blood tests. Hank is confused by the results and rushes to see Boris over Dieter's objections. He finds Boris suffering from pain when touched and some numbness. Hank needs to take more blood but Boris reacts wildly when the needle is inserted and Hank is splattered with Boris' blood. Boris tells him he has a black dot in his vision and it hurts to move his eyes. Hank is extremely confused by all these symptoms as they do not line up with the hereditary disease Boris suffers with. Suddenly Hank notices that his hands are tingling where Boris' blood touched him. He realizes Boris has been poisoned.

Dieter has a dialysis machine brought into Boris' room that can cleanse blood using charcoal filters. Hank inserts a central line into Boris and starts the machine.
At the hospital Divya asks Jane questions to help figure out her trigger. When she learns that Jane cools down in a neighbor's garden that might have legumes growing there, Divya think they have found the trigger.
As Hank monitors Boris' symptoms, he thinks the poison substance is thallium which is a lethal toxicant not found naturally in the United States. Hank calls Jill to have her laboratory verify his theory and asks her to find Divya. He also asks if she knows where they can acquire a substance called Prussian Blue, which Boris needs to survive the thallium. She checks but the closest source might be hours away.
Jill and Divya arrive at Boris' mansion to assist Hank. After a while a federal agent with a HAZMAT team arrives with the Prussian Blue which Hank gives to Boris in pill form. The agent and his team start testing everything in Boris' mansion looking for the source of the poison. Hank recommends they speak with Dieter but he seems to be absent. They also insist on testing Hank since he was exposed to Boris' blood. Suddenly Hank has an insight and asks the agent to test Boris' swimming pool.

Boris starts improving rapidly and Hank assures him he will suffer no permanent damage from the poisoning. Jill tells him she spoke with Marisa and they are fine and she gives Boris a photograph from the earlier photo session. After Jill leaves, Hank theorizes that Boris may be suffering from a mutation and not a disease. Thallium poisoning has been used for centuries between feuding factions, and his condition may be a result of long term poisoning through the generations. Boris is thoughtful and wonders where Dieter has disappeared to, suggesting Dieter had something to do with the poisoning.

Hank speaks with Divya and tells her his patient Jane's trigger was discovered by a nurse at Hampton's Heritage. He wants to send the nurse flowers but Divya tells him she will take care of it, since the nurse was her. Divya realizes she cannot keep her secret much longer.
Hank arrives home in a good mood and finds Evan pensive, worried about losing Paige. He spoke with her by phone and she said she was with her parents, but he just got a call from her father looking for her. Evan realizes he may not know her as well as he thought.
