Drew, Cole and Maya have their first day of medical school, being taught by J.D., Turk, Dr. Cox, Dr. Kelso and Denise.
J.D. and Turk tries to play cupid when they think they see a connection between Drew and Denise. Meanwhile Lucy learns the hard way that being a doctor isn't as easy as it seems.
The students look for mentors among the staff.
The interns must stay all night at the hospital to take final interviews with a terminal patient, and no one can leave until each is finished. Meanwhile, J.D. and Turk take advantage of their ladies being out of town to attend a party, trying to relive their youth.

J.D. and Turk try to figure out who gave J.D. a bad review on his last day of work. Lucy struggles with drawing blood from a live person, a requirement to continue in medical school. Drew learns to his surprise that he is officially in a relationship with Denise.

Wiht J.D. now gone, Turk looks for a new buddy on the staff to pal around with. Lucy focuses her admiration on Elliot, who seems to be able to do everything effortlessly while she is exhausted.

Dr. Cox insists that each medical student give him an honest answer about why he or she wants to be a doctor before allowing them to participate in the annual ceremony where they get their white lab coats. Lucy struggles to come up with an acceptable answer. Drew and Cole compete over being the keynote speaker at the event. Denise seeks advice from Elliot about her relationship with Drew.

As she ponders those around her, Lucy notices couples abound; her and Cole, Denise and Drew, and even a dysfunctional couple in Turk and Dr. Cox, who are still as competitive as ever.
Elliot really starts getting organized in preparation for the baby, battling J.D. all the way. Lucy's empathy gets in the way of her desire to be a good doctor as she tries to be more impersonal with her patients.

Dr. Cox finds a cheat sheet left behind in the classroom and forces the study group to stay in the classroom until one of them confesses. Turk tries to deal with a beautiful lesbian patient. Denise finds out something life changing about Drew.
Dr. Cox is jubilant because the students are about to be inundated with Hell week. Dr. Turk's glee at being head dog as Chief of Surgery is threatened by a renowned visiting surgeon. When Drew abdicates his unwanted position as study group leader, Lucy takes over with enthusiasm.
Dr. Kelso and Cole both end up on the patient side of the relationship, and both must learn to accept help and understanding from their friends.

The end of their first semester arrives and the students must present a tribute to the families of the corpse they have been using in lab. Denise decides to move in with Drew despite his fear of complicated relationships. Cole decides that surgery is his future and Turk tries everything he can think of to discourage him.
