After one of J.D.'s patients recover from cancer he tried to get the patient to stop chain smoking, which proves to be more difficult than he thinks. Elliot becomes public enemy number-one to the nurses because of her unsociable bedside manner and general attitude. Turk persistently tries to ask Carla out again and again only to get denied.

As Turk now has a girlfriend he has been spending less and less time with his best friend J.D. which has made J.D. feel left out and abandoned, to add to that J.D. suspects that Turk botched a surgery and is afraid that he himself can get into trouble for not reporting it. Elliot tries to get some help from Dr. Cox, but he is far from helpful as he does not want to be the interns' designated mentor.

J.D. treats an old lady whose kidneys are failing but gets too attached to her, and when she refuses to undergo dialysis it is quite clear what will happen. While operation on a very nice hernia patient Turk gets a big chock. Elliot and Carla are not on the best terms with each other but has to work together to save the life of a patient.

J.D. is torn when he discovers that Dr. Cox has falsely appropriated the medical insurance of a deceased patient in order to help another more needy patient, but when he inadvertently informs Cox' rival the two veteran doctors battle for J.D.'s soul during a good-vs.-evil game of golf. Meanwhile, Turk frets that his girlfriend Carla might figure out where he got his new gift for her. Meanwhile Elliot discovers exposing herself heals patients.

Elliot's patient, a psychiatrist whose jaw has been wired shut is put to the test when he has to listen to Elliot ramble and cannot do a thing about it, meanwhile J.D. has a very important patient, a hospital board member by the name of Jordan Sullivan. Dr. Cox future is quite unclear as his fate will be decided by the hospital board. Turk and Carla's relationship is put to the test when Carla blames Turk for an injury her mother sustains.

J.D. and Turk become reluctant heroes when they sneak off to a strip bar for lunch and instead save the life of a protester in front of TV cameras -- and the crafty Dr. Kelso wastes no time in exploiting their "salt-and-pepper" images in an ad campaign to tout the hospital's diversity. Elsewhere, J.D. risks Dr. Cox' considerable wrath when he insists that his mentor complete his evaluation form while Elliot feels slighted by Carla.

As Thanksgiving nears, J.D. is stricken with appendicitis and gains a new and horrifying perspective on what it means to be a patient, and to make matters worse, it's Turk who will be doing the surgery. Elsewhere, Elliot resents the fact that J.D.'s patients prefer his care to her cold hands when she has to cover his cases for him, and Dr. Cox makes sure that his former medical mentor gets proper care from the nasty Dr. Kelso.

Turk and Carla try to deal with the fact that they have trouble trusting each other and J.D. tries to fix both his relationship with Dr. Cox and Elliot, which turn out the be much more difficult than he thought.

The night they have all been waiting for, but they soon find out that it isn't as easy as they have imagined it when they are flooded with work which forces them to seek help from Carla, and even the Janitor. Meanwhile, Dr. Cox has agreed to introduce Dr. Kelso at an awards ceremony which he is not looking forward too at all.

J.D. betrays Carla by telling Turk some of Carla's feelings and insecurities towards him, while Turk has been selected to represent Sacred Heart on a trip for the "Doctors Without Borders" program, but when he finds out that his toughest rival Bonnie wasn't chosen just because she is female he isn't so sure he wants to go.

Dr. Cox's efforts to take a stand against the evil drug companies are thwarted by the seductive charm of pharmaceutical sales rep Julie Keaton. She takes advantage of the fact that Dr. Kelso has assigned Dr. Cox to coordinate a dinner reception sponsored by the drug company to pursue Dr. Cox in other arenas. Elliot and J.D. disagree about who is the better doctor, but when Elliot disregards his advice about how to treat her patient and the patient dies, it is actually J.D. that learns a lesson about taking chances. Elsewhere, J.D. and Turk take advantage of the hospital's supply closet, while taking on extra shifts at a free clinic to supplement their income as residents and Carla encourages Elliot to stand up to her father when he pressures her to pick a specialty more "suitable for a woman," especially when it will prove Dr. Kelso wrong.

Elliot thinks she has a handle on things, but when Turk is forced to take the heat from Dr. Cox for her mistakes with one of their patients, he tries to lighten her load by suggesting that her patients switch to the care of other doctors. His good intentions put him at odds with both his friends and they are forced to reevaluate their "arrangement." While Kelso defends his friend's technique, Dr. Townshend's lack of current medical knowledge forces Kelso to admit his mistake. Meanwhile, Carla tells Turk she needs time to consider his proposal because she thinks he is still too immature, prompting Turk to invite his married brother for a visit to show Carla how seriously he is. In addition, Elliot learns the sex of Jordan's baby, but Dr. Cox and his ex-wife can't agree if they want to know.

J.D., who has been watching their relationship develop, finally acknowledges that he still has feelings for Elliot and that he has to come to either accept her new relationship or do something about it. Meanwhile, Dr. Cox. is forced to swallow his pride and actually ask for help when he decides to throw his hat in the ring for the Residency Director position at the hospital. Carla picks up an extra shift as a surgical nurse, which allows her to spend the day working alongside with Turk, to his great annoyance.

J.D. and Elliot's relationship has taken a turn for the worse when J.D. is accidentally called the co-Chief Resident and Elliot the Chief Resident. This mix-up lifts Elliot's post break-up spirits. Dr. Cox and Jordan are having relationship problems as well when they find out that their divorce was not final. Meanwhile, Carla visits Elliot at her new job and inadvertently creates an awkward new nickname for her friend when she tells her colleagues an embarrassing story. When J.D. is asked to introduce Dr. Kelso at an awards dinner, he sets out to discover something good to say about the grouchy doctor.

Meanwhile, Dr. Kelso and Dr. Cox each have a patient whom they feel deserves the benefit of a new experimental treatment, but a moral standoff ensues when there is only one open slot in the program.

J.D. is called in to work on his day off and every time he tries to go home, he is pulled into another drama. Turk searches for a heart donor to save a dying patient, and Carla is worried that she doesn't have the courage to become a parent.

Meanwhile, Elliot doesn't think she has the brains to lead a question and answer session on a subject she knows little about. All the while the Janitor is trying to make the hospital easier to navigate by painting color-coded lines, including a yellow line, which leads to the exit.

When J.D.'s underachieving brother Dan returns for a surprise visit, Elliott learns that J.D. has been telling Dan that she's still into him, but when the truth comes out, Dan turns to the Janitor to get back at J.D. As everyone reacts to the pregnancy in different ways, Carla and Turk begin to discuss baby names, and despite his promise to Carla, Turk is unable to keep quiet about their initial choices. After assigning someone else to assist "sweaty Teddy" with incoming malpractice claim paperwork, Dr. Cox is caught off guard when Dr. Kelso steps in to stick him with the job. Meanwhile, a patient with weight-related problems needs an MRI, but the staff has to come up with a solution when they find he's too big for the machine.

Meanwhile, when mistakes by the interns begin to reflect badly on her, Elliott cracks the whip on her charges and even resorts to using her mother Lilly to lean on Keith for information about his peers. Elsewhere, Carla discovers an unlikely ghost haunting pediatrics in an attempt to keep it clean. In the end one of Sacred Hearts own will be considering a marriage proposal.
