It has been three years since Sherlock Holmes's supposed death with arch enemy Professor Moriarty at the Reichenbach Falls and Dr. Watson, although continuing to grieve for his friend, has set up a Medical Practice in London, acting as Police Surgeon for Scotland Yard.

When young Ronald Adair is murdered, Watson encounters an elderly man outside the Police Courts after the inquest into the death. The man follows him back to Watson's lodgings and Watson is amazed and delighted to find that it is Holmes himself, heavily disguised but very much alive and keen to investigate the Ronald Adair case.

The game is afoot once again!
