Holmes is taking a rest cure in Cornwall with Watson when the strange case of the Tregennis family is brought to his attention. Brenda Tregennis is found dead one morning with a look of terror on her features; her two brothers, George and Owen are quite insane with the same appearance of a deathly fear on their faces. The family had been playing cards with their other brother, Mortimer, who had left the cottage at an earlier time.
Later, Mortimer is also discovered dead from the same strange cause. Holmes investigates and discovers the truth.
While investigating this case, Holmes gives up his cocaine habit.
