In 1972, Father Lehne is at St. Mary’s Convent when a black demon-cloud enters the building and possesses him. Later, he gives a sermon to the convent’s nuns and talks about his “father,” who is imprisoned beneath the convent. The possessed Lehne says that his father was once an angel and someone sensed the holiness and built the convent there. He then grabs a large blade and tells the nuns to pray, as his eyes turn demon-yellow.

In the present, Sam and Ruby are preparing to leave their haven. Ruby says that Dean was wrong for accusing Sam of becoming a monster, but Sam figures that Dean isn’t to blame. She says the brothers will be able to patch things after they stop the Apocalypse, but Sam wonders if there will be an “after” and then leaves.

Dean is with Bobby and explaining what happened. Bobby insists Dean needs to try again to get through to Sam but Dean insists that Sam is beyond help. Bobby snaps at him, telling him that you never give up on family, and that John Winchester was a coward for shoving Sam away instead of reaching out to him. He tells Dean not to be like John. Dean considers what he’s said… and then finds himself teleported into a room in a luxurious white mansion decorated with religious iconography. Castiel appears and says that it’s almost time.

Lilith’s demon chef has taken over the body of a nurse at a maternity award. She steals a baby but Sam and Ruby arrive and Sam immobilizes the demon, saying they need to talk.

A banquet table filled with hamburgers and beer appears in the mansion and Zachariah appears to offer Dean whatever he wants. He says they’re keeping Dean in the “green room” to keep him safe until the final show down. Dean demands answers and Zachariah explains that there is only one seal left to be opened. When Dean gets sarcastic, Zachariah points out that he’s the one who started the last thing. Zachariah explains that Lilith will open the last seal at midnight, but refuses to answer any other questions and reminds Dean that he swore an oath of obedience to Heaven.

Sam uses his powers to torture the demon-nurse. The demon insists she has no reason to talk because Sam will kill her no matter what and Lilith will kill her if she gives away Lilith’s location. Sam tells her to worry about what happens before she dies and tortures her further.

Dean decides to call Sam and leaves a message apologizing for accusing him of being a monster.

The demon-nurse finally breaks and gets Sam to promise to kill her once she’s told everything. She reveals that Lilith is at the convent in Maryland. She doesn’t know what the final seal is. Ruby tells Sam to keep the demon alive so he can use her blood to power himself for the final showdown and Sam agrees, breaking his promise. When the demon-nurse realizes Sam plans to feed on her, she withdraws into her host’s subconscious and lets the human personality, Cindy McClellan, emerge. Cindy has no idea what’s going on and begs for them to release her.

In 1972, the yellow-eyed demon, Azazel, prays to his father Lucifer after disemboweling the nuns. Lucifer speaks through a dead nun, telling Azazel that he must find Lilith. Azazel warns that Lilith is imprisoned in Hell and Lucifer explains to free her, Azazel must find a special child.

In the present, Sam checks the Internet and discovers that Fehne went berserk and claimed he was possessed by a demon, and the convent has been closed ever since. When he proves reluctant to kill Cindy and the demon inside of her to get its blood, Ruby tells him that the demon is playing with him. Sam, resolved, locks Cindy in the trunk and contains her with a demon’s trap.

Castiel comes to see Dean, and Dean asks to meet with his brother to resolve a few loose ends. Castiel warns against it and notes that things went badly the last time Sam and Dean went. Dean insists but when the angel refuses, Dean tries to walk out only to discover the door has vanished and Castiel with it.

Sam and Ruby drive toward the convent with Cindy screaming in the trunk. Sam sees that Dean has left him a voice mail but can’t bring himself to listen. Ruby tells him to suck it up and wonders if he’s having doubts. She insists that he’s killed demon-possessed innocents before and this time it’s worth it to stop the Apocalypse. Sam begins to wonder if Dean was right all along but Ruby says he has to follow through.

Dean tries to break through the mansion’s wall but the hole repairs itself. Zachariah arrives and Dean demands to see Sam. The angel refuses and finally admits that there’s no way to stop Lilith from opening the final seal. Dean realizes that Zachariah never intended to stop Lilith and the angel admits that Dean is right. Zachariah explains that he’s just tried to stop the opening of the seals for the benefit of the lower-rank angels. In reality, he wants the Apocalypse to happen since the angels will win and there will be Heaven on Earth… at the cost of a few million lives. Sam has his own role to play, and so does Dean: defeating Lucifer. Dean wonders where God is in all this, but Zachariah says that God is no longer involved before disappearing.

Lilith and her demons appear at the convent and capture the security guard.

As Dean tries to call Sam, Castiel appears to tell him it’s no longer allowed. Castiel says that they aren’t making Sam do anything, that he’s doing it all to himself. The angel apologizes to Dean for how everything is turning out, and Dean punches him without effect. Unable to hurt Castiel physically, Dean tells him that he’s being used by the upper angels and he’s an unemotional bastard. Castiel wonders what Dean wants to save, pointing out all the guilt and anger he’s seen in humans. Once they’re dead, they’ll be at peace in Heaven. Dean says he’d rather have the anger and guilt then live in peace, and asks Castiel for help. Castiel wonders what Dean wants him to do, and then warns that if he helps they’ll all be hunted and killed. Dean says there’s nothing better to die for, but Castiel disappears.

As Sam and Ruby near the convent and park for a minute, Sam asks for a minute to consider. He plays back Dean’s message but it’s been tampered with, and “Dean” says that Sam is a monster. Sam tells Ruby to get on with it, and the demon goes back to start bleeding Cindy.

Castiel returns to Dean, cuts his own arm with a knife, and then uses the blood to draw a symbol on the wall. Zachariah arrives to demand answers and Castiel completes the symbol, sending his superior back to Heaven. He warns that Zachariah will son be back and they have to find Sam: Lilith is the final seal, and if Sam kills her then Lucifer will be released. Castiel prepares to take them to the one man who knows where Sam is: writer Chuck Shurley, who has written his final novel, “Lucifer Rising.” He’s busy ordering a bevy of strippers when Dean and Castiel appear. Chuck says that they’re not supposed to be there and Castiel says that they’re improvising.

Lilith is at the altar at the convent and her demons bring her the guard’s blood. She tells them they’re going to save the world, but when she turns back to the hallway she discovers that Sam has destroyed them all. She slams the door closed before he can get in.

Chuck tells Dean and Castiel that Sam is at St. Mary’s Convent. A blinding light appears as the Archangel comes to stop them. Castiel sends Dean to the convent while he stays behind to stop the Archangel and the hosts of Heaven from following. Angel and writer face the light together.

Using his enhanced abilities, Sam breaks through the door and telekinetically immobilizes Lilith. Dean arrives in the hallway outside but Ruby sees him and seals the door before he can get in. As Sam destroys Lilith, he hears Dean pounding on the door. Ruby tells him to finish Lilith, while Lilith taunts him for being a monster who can’t finish the job. Sam focuses all of his remaining energy on Lilith as his eyes turn black, and he kills her. Lilith’s blood oozes out and forms a symbol on the floor. Ruby, triumphant, explains that Sam has freed Lucifer, and that she set the whole thing up. Sam is out of power to stop her, and Ruby explains that it’s a miracle that Sam has succeeded after all the effort she made to manipulate him. No one knew about her plan except Lilith, and she promises Sam that Lucifer will reward them both when he’s freed.

Dean breaks through the door and comes at Ruby. She tells him he’s too late but Sam grabs her and Dean kills her with the demon-killing knife. The brothers look on as the blood symbol on the floor breaks wide open. As white light fills the convent, Sam says that they’re too late and “he” is coming…
