On the outs with Heaven, Zachariah goes to a bar on Earth to have a drink and commiserate with an unemployed human. They complain about their respective bosses and outsourcing, and Zachariah complains that he failed to close the deal. The bar suddenly shakes and Zachariah informs his drinking partner, Stuart, that his boss is coming. A high-pitched whine fills the bar and the human occupants clutch at their ears in agony. Zachariah tells his boss to get on with it, but realizes that he's been given a second chance. He thanks Michael profusely and leaves, stepping over the human corpses.

Dean is sending his gear to Bobby and drinking when Sam comes in and accuses him of giving in to Michael. Dean says that Sam has given up plenty of times and run away, and Sam admits that every time he was wrong. When he half-heartedly insists that Bobby is coming up with a plan, Dean calls him on it and Sam says that he still has to stop Dean. When Dean dares him to do it, Sam says he brought help and Castiel renders Dean unconscious.

Dean wakes up at Bobby's house and finds Bobby, Sam, and Castiel trying to find a plan. When Bobby asks what the matter is, Dean tells him that he's not his father and doesn't know what Dean is going through. Bobby responds by taking out a revolver and a bullet, and explaining that it's the bullet he would use to kill himself, except for his promise to Dean that he wouldn't give up. Castiel suddenly winces in pain and tells them that something is happening. He teleports to a forest and finds something buried in the ground. Angels attack him with mystical knives and Castiel manages to defeat them both. A hand reaches out of the ground and Castiel pulls out the person and teleports away with him.

Castiel returns to Bobby's house and shows them the person he recovered: Adam Milligan, Sam and Dean's half brother. The angel puts Enochian concealment sigils on Adam's bones to conceal him from Heaven, and Sam and Dean tell him who they are. Adam explains that Zachariah already told him all about them. He tells them that after the shapeshifters killed him, he went to Heaven but the angels told him that he was to provide a vessel for Michael so they could save the world and win the Apocalypse. Castiel admits that since Adam is part of John Winchester's bloodline, the plan could work. Sam tries to persuade his half-brother that the angels lied to him, and that they're brothers. Adam is unimpressed, saying that John all but ignored him and that if accepts Michel, he'll be reunited with his mother in Heaven. Sam admits that he's right about John but asks for a little time before he decides.

Later, Adam tries to sneak out but Sam stops him and Adam realizes that he's on lockdown. Sam tries to convince him that John was trying to protect him by staying away, but Adam would rather have had more of John's time. Sam says he might have been lucky not too, and that they would have found him if they knew he existed. When Adam snaps at him, Sam admits that he has the typical Winchester attitude.

To keep Dean from leaving, Sam and Castiel lock Dean up in Bobby's anti-supernatural panic room. Dean wants to keep Adam from sacrificing himself by accepting Michael himself, and figures it's his destiny to give in. When Sam wonders what has happened to change his mind, Dean admits that he no longer believes in Sam. He figure that Sam will eventually accept Lucifer, and Dean needs to be there as Michael to stop him. Shocked, Sam leaves him alone.

Later, Castiel is watching over Adam and goes to check on Dean. He hears the sound of breaking glass but can't see Dean through the panic room's door-window. He goes inside and Dean is ready with an angel-banishing sigil written in his own blood. Dean sends Castiel away and leaves. When Sam realizes that he's gone, he tells Bobby to watch Adam while he tries to find his brother.

In bed, Adam dreams that he's at a childhood park waiting for his mother. Zachariah appears on the bench next to him and tells him that his mother will be there some day. The angel figures that Adam is with Sam and Dean but shielded from detection. Zachariah warns the teenager that the Winchesters can't be trusted, and warns him that they'll protect themselves first and say anything to turn Adam against the angels. When Adam has second thoughts, Zachariah threatens to keep him from his mother, and Adam wakes up.

Dean goes into the city and finds a street preacher to tell the angels he's ready. However, Castiel arrives, renders the man unconscious, and drags Dean into an alley to beat him unmercifully. As he does, Castiel yells that he's given up everything to protect Dean, who is now giving in and giving up. Dean finally just tells the angel to kill him, but Castiel renders him unconscious again and teleports him back to Bobby's house, where Sam has learned that Adam disappeared. Castiel explains that Adam let the angels take him, and he knows where.

Adam is in the beautiful room enjoying cheeseburgers and beer when Zachariah comes to see him. The angel tells him that they don't need him and never did: his role is bait so they can get Dean there and force him to accept Michael. The plan is that the brothers will unite to save Adam and they can finally get Dean to accept his destiny. As an angry Adam starts to question him, Zachariah uses a gesture to cause him to cough up blood.

Dean finds himself back in the panic room with Sam. His brother tells him that Castiel has scouted ahead and confirmed that Adam is in the beautiful room. He uncuffs Dean and explains that there are too many angels guarding their half-brother and they need Dean's help. Dean wonders why Sam is releasing him just to give in to the angels, and Sam says that he knows Dean will make the right decision when the time comes, and because Dean is his big brother.

Castiel teleports Sam and Dean to the beautiful room, which is inside a muffler warehouse in Van Nuys, California. As they prepare to go in, he explains that he's going to distract the five angels guarding Adam while Sam and Dean get the teenager out. He admits that it's likely he'll be killed, but he'd rather die then watch Dean fail yet again. As he goes in, Castiel takes a box-cutter knife out of his pocket and Sam wonders what it's for.

As Castiel approaches the office inside the warehouse, one angel attacks him. Castiel disposes of him with a mystic blade but four more surround him. He dares them to do their worse and then rips open his shirt. Castiel has painted an angel-banishing sigil on his chest in his own blood, and activates it. He and the four angels are sucked back to Heaven, and Sam and Dean come in. They go into the beautiful room but Zachariah is ready for them. He casually tosses them both aside and tortures Sam and Adam, and demands that Dean yield. Dean agrees in return for an end to the torture. Suspicious, Zachariah summons Michael. As they wait, Dean winks at Sam and says that he has one more condition: that Michael destroy Zachariah. Zachariah insists that he's too important. As the room glows with a white light and a high-pitched noise fills the room, Zachariah grabs Dean and promises to make him suffer... and Dean stabs him in the head with a mystic knife.

As the sound and noise reach a crescendo, Dean gets Sam out with Adam right behind. However, the door slams shut before Adam can get out and he turns to face the light. Outside, Sam and Dean see the light disappear. They turn the knob and find it unlocked, but discover that the beautiful room has now reverted to its original form of an abandoned office, and Adam is nowhere to be seen.

The brothers hit the road and they wonder if Adam is okay. Dean promises to find Adam and Castiel. Sam then asks why Dean changed his mind and said no to Michael. Dean explains that he couldn't let his little brother down, He apologizes and explains that if Sam can have faith in him, then he can have faith in Sam to do the right thing and refuse Lucifer. They both decide to ignore their supposed destiny and take the fight to the angels.
