In Buffalo, New York, a businessman leaves a bar and goes out to his car, while calling home to check on his car. He gets in, unaware that something him from the bushes. When Ron prepares to start the car, something leaps out, crashes throuigh the windshield, and rips open his chest.
Sam and Dean are eating at a ribshack, and Dean calls Bobby to try to find a way to get Sam's soul back from Crowley. Crowley appears and tells them that he has a job for them. When Dean refuses, Crowley demonstrates that he can infect Sam with Hell by simple touch. He informs Dean that he owns Sam, and that what they have is a hostage situation. He tells Dean that if they bag him a live alpha, he'll give them Sam's soul back. Crowley shows them a newspaper article about the dead businessman, and they figure it's a werewolf. Sam warns Dean that werewovles have been appearing during other phases of the moon. Crowley wishes them well and disappears.
As they drive to Buffalo, Sam determines that the businessman was a slum lord. Dean is angry that Sam is just going about it as normal, and Sam admits that it's a mess, but he's still the same Sam. Dean isn't convinced, but Sam says that he just wants to prove he's the same old sam.
The brothers arrive Buffalo and discover that another werewolf murder has occurred on the docks. They pose as FBI agents and talk to the detective, who confirms that the second victim's heart was ripped out. He insists that it was an animal attack, but Dean notes that they're ont he docks.
The next morning, Dean awakes and discovers that Sam has been up all night since he doesn't sleep anymore. He explains that he tracked the two victims and determined that they are connected through Cal Garrigan. They go to Cal's home and his girlfriend, Amanda Duren, lets them in. She's tending to her son, Aiden. They ask to talk to Cal and she lets them in. He staggers in past the family dog, Lucky, and claims that he was out all night drinking. They figure he blacked out and doesn't remember what happened. When Cal wonders what's going on, they explain that they've investigating the death of Cal's brother, Ron. Cal admits that relations between him and his brother weren't good, and he called the cops the last time that Ron came by. Sam then notes that their landlord sent eviction papers, and he was the first victim.
Outside, Sam suggests they bag Cal now, but Dean says they have to be sure before handing him over to Crowley. Sam is surprised Dean wants to make sure. That night, they follow Cal to a series of bars but there's no indication that he's a werewolf. When morning comes, the brothers take a break. Once they're gone, Cal goes to his car and is surprised to see his family dog, Lucky,waiting for him. The dog leaps at him and rips out his heart... and then transforms into a naked man. The man goes to Amanda's home, enters her bedroom, and transforms back into a dog. Lucky gets into bed with her and watches over her. When Amanda wakes up, she realizes that Cal didn't come home that night. She hugs Lucky and admits he's the only decent boyfriend she's ever had. As Amanda goes to the shower, Lucky watches her.
The Winchesters go to the new crime scene and figure that something more than a werewolf is involved since Cal was killed during the day.Sam points out that Amanda is the only remaining suspect, and casually asks Dean if he can give her to Crowley. Dean reluctantly admits that he can.
Amanda tends to Aiden, who has the flu, and then takes Lucky for a watch. She notices blood on its fur but assumes that the dog killed a bird. Sam and Dean approach her on the street and inform her that Cal is dead. They take her home to explain, and then ask her to come with them to answer some questions. Amanda asks for a delay because Aiden is sick, and Dean wonders if she with the son all night. He asks to talk to Aiden, although Sam doesn't see the point. Afterward, Dean figures that Amanda is innocent, but Sam doesn't believe it. He offers to watch Amanda while Dean checks out the crime scene. Dean is suspicious, but Sam insists that all he's going to do is watch her.
That night, Sam watches the house and sees Lucky transform from to human. Lucky dresses and then goes to a nearby playground and meets with another man. As Lucky goes back to the house, Sam prepares to shoot him, but the shapeshifter smells him and runs. As he goes into the woods, Lucky transforms back into a dog. However, when he gets to the road, he's hit by a passing SUV. Before Sam can get there and claim it's his dog, the driver picks up Lucky and drives to the vet.
At the motel, Dean considers calling Lisa but then puts the phone down. Sam calls to tell him what happened, and that they're up against a skinwalker. He's called Bobby, and determine a skinwalker is similar to a werewolf. The next morning, they go to the pound with sivler bullets and chains, and force Lucky to go with him. At the motel, they tie him up in human form but he refuses to talk. Sam threatens to torture him, but Dean intervenes and says that he figures Lucky just protected Amanda and Aiden. He wants to know who Lucky met with. The skinwalker balks, but Dean warns that Amanda and Aiden will soon be in danger and they want to help her. Lucky admits that the guy is a skinwalker, and that there are 30 other people who were recruited and transformed into skinwalkers with a bite. He was homeless and had nothing better. Their orders are to infiltrate families, then wait for word to rise up and change their families, all in one night. Lucky explains that there is a pack leader, but he's pretty sure it's not an alpha. Dean asks for his help to stop them, but Lucky refuses until Dean points out that he won't be able to bring himself to kill them.
As they prepare to go to the meeting place, Dean reveals that he plans to shoot the pack leader with a sniper rifle so they don't get scented by the skinwalkers. Sam points out that if they kill the pack leader, they lose the trail to the alpha, putting his soul at risk. Dean points out that if they don't kill the pack leader, 150 people will be transformed. He then tells Sam that he's clearly not himself any more, and tells him not to stop pretending.
Lucky goes to a garage while Dean and Sam set up on a rooftop. As they wait, Sam wonders if Lucky will betray them, and admits that he would in Lucky's place. The pack leader arrives in a SUV, and more skinwalkers emerge from the garage. Sam recognizes the driver from the park, accompanying the pack leader. However, the skinwalkers get a captive Amanda and Aiden out of the van and take them inside with Lucky. Dean refuses to shoot because Amanda is in the way, and Sam tells him to shoot anyway. Everyone goes inside and the Winchesters try to come up with an alternate plan.
The pack leader's lieutenant tells Lucky that he screwed up by commiting unsanctioned murders. Lucky insists that he's with the pack, and the lieutenant tells him to turn Amanda and Aiden while they watch. If they don't, the lieutenant will kill them all. The lieutenant smells Sam as he comes in and shoots the pack leader dead. Dean provides cover fire from above while Amanda crouches behind cover. Lucky offers to help her, but she doesn't know who he is. As he leads her away and locks her in an office, Sam goes after the other skinwalkers, who transform into dogs. One goes after Dean, who shoots it with his handgun.
The lieutenant approaches Lucky and wonders how he could have betrayed them for the humans. Dean prepares to shoot him, but Lucky punches him. The lieutenant, unimipressed, says that he's going to kill Amanda and Aidan instead of turning them. Lucky transforms into a dog as Amanda looks on, shocked. Unimpressed, the lieutenant pulls out a gun and wounds him with a silver bullet. He moves in to fire a final shot... and Dean gets a clear shot and takes it. Sam finishes off the other skinwalkers and sees the wounded Lucky, and prepares to kill him. However, by the time he gets there, Lucky has slipped away. Sam looks at the shocked Amanda, unconcerned.
Later, Lucky comes to see Amanda and tries to explain. He tells her that they're the only family that he ever had, and that no one had ever been nice to him before. He thanks her, but Amanda tells him to get away from the house and never to return. Lucky transforms back into a dog and walks away.
Sam and Dean go for lunch in the park, and Dean wonders how many other packs are out there, waiting for the signal. Sam then admits that Dean was right, and he's not Sam. He doesn't care anything about Lisa and Ben, or Dean, or anyone. He only puts up with Dean because he can help get his soul back, and Dean won't stay unless he's honest. Sam admits that he's killed innocents in the line of duty, but the old Sam wouldn't have done it. When Dean wonders where he's going, Sam says that being the old Sam was harder, but he'd rather go back to being him. Dean admits that it's a start and agrees to continue working with him and do what they have to do.
