Sobbing, Dean tries to rouse Sam out of his coma. The memories of burning in Hell finally fade away and Sam wakes up.
At the Great Falls Junior College in Paterson, New Jersey, Steve the janitor is working in the biology lab, while an anatomy dummy hangs nearby. Its eyes turn to follow him, but when Steve turns around, it has disappeared. As he mops, Sam looks down and sees blood dripping on the floor: he has a head wound. The temperature drops and the lights go out, and a tray of scalpels clatter to the floor. Steve runs for the door and the mannequin dummy appears in front of him. It cuts his throat with one of the scalpels and returns to its place on the hook.
Sam and Dean stop for breakfast, and Sam insists that he's fine. He admits that it felt like he was out for a week or two, but insists that he's fine now. Dean doesn't believe it and warns that Sam could have died. Now they know it's a risk, and he tells Sam to ignore his past memories. Sam insists that he has to make amends for what he did, but Dean tells him to shove the memories down debate, and Sam reluctantly agrees. Dean suggests they check out Steve's death the previous night, in a locked lab.
The brothers go to Paterson and check out the lab. Dean smells sulphur, but Sam notes they're in a science lab. Dean gets a call from Lisa and rejects it, and Sam suggests he return her call. Dean tells him to man his own business, and Sam picks up EMF transmissions. They check the security footage and discover that the film fuzzed out at the point where Steve was killed. They go to see Steve's girlfriend, and Dean waits in the car while Sam confirms that Steve went to church and had a spotless record. Dean gets another call from Lisa and ignores it. When Sam returns to the car, Dean confirms that there's no sign of supernatural incidents at the college lab.
That night, a watchman, Dave, is patrolling the Salzman & Sons garment factory three towns over from Paterson and hear something moving around. He figures that his friend Jonny is playing a prank on him, but gets no response. He turns around and finds himself face-to-face with a mannequin. Dave watches as his breath freezes in the air and a cut opens on his forehead. He turns to go and another mannequin cuts off his escape. It raises a pair of scissors and lunges at him, and Dave trips. The other mannequin impales him with a metal rod, killing him.
The next day, the brothers check out the new murder scene and Sam picks up EMF readings from the mannequins. Sam realizes that the EMF readings at the lab came from the anatomy dummy, Dean doesn't believe it, but admits that he's creeped out by the mannequins. He also notes that ghosts don't jump cities, but figures they have to check it out. They go over the company files and determine that Dave had a clean reputation. Sam discovers that a seamstress, Rose Brown, went missing a year ago and was last seen at the factory. She's survived by a sister, Isabel. As they leave to talk to her, Lisa calls again and Sam tells his brother to answer it. It's Ben, who says that something's wrong with Lisa and she's locked herself in her room. Sam insists that he can handle it and sends Dean on.
Sam talks to Isabel, who explains that Rose was a nice girl but shy, and some people gave her a hard target. Isabel notes that Rose did more for her than anyone else ever could, and she misses her. He notices a photo of the Salzman & Sons Christmas party with Rose, and notices that Isabel, Dave, and Steve are also in the photo. Sam calls and tells Dean what he's learned, and that Steve quit the factory a year ago, right after Rose disappeared. They're still not sure what the connection is, so Sam goes to the factory posing as an FBI agent and talks to the employees. One of them, Jonny, acts blatantly suspicious. He claims not to know Rose until Sam prods him repeatedly to remember.
Dean arrives at the door of the Braeden house and discovers that Lisa is fine... and going out on a date. He realizes that Ben faked an emergency to get him there, and asks Lisa who she's dating. When Dean finds out that it's a doctor, he gets defensive and Lisa points out she called him six times. Dean says that he dropped everything when he heard she was in trouble, and asks what she wants. When she says she doesn't want anything, Dean tells her to ask for something. Lisa says that she can't have the one thing she wants, and she can't bear the thought of Sam calling to tell her that Dean is dead. She asks Dean what he wants, but he doesn't have an answer.
Jonny goes to the factory and tells a friend that he's flipping out because Steve and Dave are dead. As he hangs up, his forehead starts to bleed. The temperature drops and he finds a mannequin in the room with him. Sam arrives, gets him into an isolated room, and uses salt to seal the doors and windows. He then explains that Rose is back as a ghost and he needs to know what they did to Rose.
A year ago, Jonny, Steve, and Dave played a practical joke on Rose. They made her think she had a secret admirer and lured her to an apartment. The pranksters set up a mannequin as her admirer, and then jumped out to laugh at her. As Rose tries to leave, Steve grabbed her, she pulled away, and she hit her head on an end table. The blow killed her, but when Jonny tried to call the cops, Steve warned him that they' at least go to jail on involuntary manslaughter. They took Rose's body out into the woods and buried it.
Jonny insists that he wasn't responsible, but Sam doesn't believe it. However, he insists on helping the worker and gets the address where Rose's body is buried. Sam tells Jonny to stay inside the salt while he deals with the body.
Dean goes up to see Ben, who refuses to apologize and insists that it was an emergency. He suggests that Dean apologize, but Dean says that he can't. He assures Ben that the boy didn't do anything wrong, but he's not someone they can have at their dinner table. Dean tells Ben that if he stays around, Ben will end up with just one path in life. Ben calls him a liar and says that he's walking out on his family.
Sam finds Rose's grave and burns the salted bones. He then calls Jonny and says that he's safe, and he has a chance to not be a jackass.
As Dean drives home, he remembers all of his time with Lisa.
Isabel and some friends from the factor go to McOwen's Bar. Jonny pulls up outside and goes to his apartment above the bar. Waiting for him is a mannequin, Jenny, dressed up as his girlfriend. Jonny tells her to pack up and takes her hand, and then stares in horror as it comes to life.
The next day, Sam learns that Jonny was strangled in his apartment. He sees the mannequin and realizes what happened, and calls Dean to tell him that it's not over. He heads for Isabel's and tries to find out what else of Rose's has survived. Isabel says that she gave most of her sister's clothing to Goodwill. Sam notices a chemistry book and realizes that she goes to Great Falls Junior College, and has been there and at the factory that week. He also confirms that she was at McOwen's, When Sam realizes that she's the connection and asks what she has of Rose's, Isabel explains that her sister donated a kidney to her when she was sixteen.
The brothers meet at a parking lot and try to figure out what to do. They can't remove the kidney without killing Isabel, but if it remains then Rose will be able to take vengeance on anyone else who crosses her path. Dean suggests that they call Dr. Robert, but Sam wants to head for Louisiana to find some hoodoo. Isabel overhears them and demands an explanation, but they're interrupted when the Impala comes to life as Rose possesses it. It tries to run over Dean, who runs for a storefront. He ducks aside at the last minute and the Impala slams into the window. The glass shatters, and a piece impales Isabel. As she lies dying, Rose's ghost appears and apologizes, and then disappears for good as Isabel dies.
Later, the brothers take the Impala to Bobby's auto yard to make repairs. Dean wonders what they've accomplished, and Sam admits that the hunt wasn't a win. He admits that they've had some bad luck, but he has his soul back and thanks Dean for what he did. Sam assures Dean that he has his back, and Dean says that he knows.
