At Crowley’s laboratory, Sam, Dean, and Bobby stare at the newly omnipotent Castiel. Bobby immediately bows down and tells the brothers to do likewise. As they do, Castiel says there’s no point if they do it out of fear. Sam tries to appeal to him but Castiel notes that he stabbed him in the back and has nothing to say to him. Dean says that Castiel isn’t acting like himself and the former angel says that he is no longer the individual they knew, and that he is now God. If they rise up against him, he’ll strike them down. Sam staggers and Castiel says that he won’t fix his memories since they rose up against him, and then teleports away. Sam starts to remember more of his time in Hell and collapses.
Castiel goes to Heaven and addresses the angels. He admits that he thought the answer was free will, but now he understands that they need a father, and he is their father now. Castiel tells them to be obedient and says it is a new day, leaving a field of dead angels around him.
Back at Bobby’s junkyard, Dean works on the wrecked Impala. Bobby comes out and reports that Sam is still out of it. He hasn’t found out anything they can use against Castiel and Dean admits he doesn’t know what to do either. Dean snaps at his friend, telling him to figure it out, and admits that there’s nothing they can do against God. He sticks to doing what he can do, repairing the Impala. When Sam wakes up, he plans to glue him together as well. When Dean goes inside, he finds Sam, seemingly back to normal. Dean invites him outside to work on the Impala and fill him in. As Sam goes after him, he hears the echo of rattling chains.
A preacher is speaking to his congregation against homosexuals and claims to speak to God. Castiel appears and says that he’s indifferent to sexual orientation, but he hates hypocrites. He declares himself as God and one man tries to stand up, glances at him and forces him back down. Castiel then tells the preacher to choke on his own false tongue and he does. As Castiel leaves, declaring himself God, he hears a voice in his head calling his name and grabs at a pew to steady himself. The wood burns beneath his hand and he continues on.
Sam is getting tools from Bobby’s basement when he hears chains rattling. When he turns around, the basement is filled with hooked chains. Sam grabs a tool and swings it around, but Bobby comes down and Sam realizes that he’s hallucinating. Bobby calls him upstairs where the TV news is reporting the deaths of hundreds of religious leaders across the country, followed by white-supremacy members and New Age motivational speakers. Sam says that they should try talking to Castiel but Dean says that their former friend isn’t coming back and there’s nothing they can do but get out of the way.
Crowley is holed up in an RV listen to a newscast about the healing of a leper colony when Castiel teleports in, easily bypassing Crowley’s protective wards. The demon dares Castiel to do his worst, but Castiel says that he’ll restore him to his post as king of Hell. However, Castiel will control the flow and Crowley will take whatever is given. He needs Hell as a threat against his enemies and to hold Michael and Lucifer. Crowley accepts, calling Castiel “boss,” and Castiel glances at his hand and realizes that the skin is burning. He says he’ll be in touch and teleports away.
Sam is resting when a chain burst through the ceiling, grabs his throat, and lifts him up into the air. He finally wakes up from his nightmare and realizes that nothing happened. In Bobby’s garage, Dean finishes restoring the Impala and shares a beer with Bobby. They discuss Sam and Bobby wonders how he can still be standing. Dean insists that his brother is fine at first but finally admits that they never catch a break and he doesn’t plan to get his hopes up. Sam comes out and overhears them, and then updates them on Castiel’s smiting. He wonders if they could use something from the angel arsenal to hurt Castiel, and Dean says there may be someone who can help them.
The trio summons Crowley and tells them that Castiel will kill him for working with them. They ask for a spell to summon Death and figure that they have to be handle him because he’s the only option left. Crowley is reluctant to agree but realizes they have no choice.
Castiel walks in the world and finds a blind beggar. He insists that he’s not just about wrath and heals the man with a touch. The man thanks Castiel and tells him that his face is burned. Castiel finds a mirror and as he examines his flesh, hears voices in his head, demanding that he release them. Hands try to burst through his chest and Castiel contains them with an effort.
Crowley smuggles an envelope with the Death-binding ritual to Bobby. They have all the components except a fulgurite, a piece of sand crystallized by lightning. Bobby has found the winning bidder of such a piece in an auction, Dr. Weiss. They break into Weiss’ house, subdue the doctor and his wife, and find the fulgurite. They begin the ritual and the house shakes with the power of the spell. Death appears within the circle and holds up his bound hands. He warns that he can’t do anything further about Sam’s hallucinations, surprising Dean since he didn’t know about them. They finally tell him that they need Death to kill God, and Death wonders why he should. He is unimpressed with Dean says that they can control him.
Before Death can give them an answer, Castiel appears and prepares to kill them. However, Dean notes that he can’t kill them as long as they control Death. Death dismisses Castiel as a mutated angel and warns that his vessel will soon explode, and Castiel insists that he’ll finish his job and repair himself. However, Death says that there are things older than souls in Purgatory and they are inside of Castiel as well. When Dean wonders what he’s talking about, Death explains that God created the First Beasts, the Leviathans, but then banished them to Purgatory rather than let them destroy Earth. Now Castiel is the only barrier between the Leviathans and Earth. Castiel insists that he did a service by cleaning up all the messes that God left behind and threatens to destroy Death. They argue back and forth until Dean tells Death to kill Castiel. Death agrees but Castiel frees him with a snap of his fingers. Freed, Death picks up Dean’s pickle chips as Castiel teleports away.
Senator Michelle Walker is running for reelection and Castiel teleports in. He explains to an aide that he is there to punish a woman who causes poverty and despair in his name. Castiel yells at everyone in the room, insisting that he’s a better God than his father, and then laughs hysterically as his features continue to burn.
Death finishes his food and tells Dean that he should have stopped Castiel long ago, and threatens to find a better planet. As he leaves, Sam asks him for help and Death tells them to have Castiel return the souls and Leviathans to Purgatory. If they can get him to Crowley’s lab and compel him to return the souls, things will be fixed. When Bobby points out that the door only opens during an eclipse, Death tells them that he’ll make another one and gives them a precise time. As he leaves, he tells Dean that if they try to bind him, he’ll die before he starts.
Castiel wakes up covered in blood and surrounded by the corpses of the senator and all of her staff.
Back at Bobby’s, Sam tells Dean that they need to hit the road but Dean says that they can’t succeed and he’s giving up. When Sam says that Dean has never given up on him and shouldn’t give up on Castiel, Dean point out that Sam hid the hallucinations from him. Sam tells his brother that he couldn’t help and insists that it’s under control, but Dean doesn’t believe it and prepares to relax before the end. As he turns on his laptop, he sees a broadcast of the security footage from the senator’s headquarters. On the footage, Castiel is grinning maniacally and looking directly at the camera, and Dean figures there’s no way to reach him.
Sam goes outside and calls to Castiel, insisting that the angel is still one of them and asking him to come back and let them help. He goes back inside and shares a drink with Dean, and Castiel teleports into the room. He says that he heard Sam’s call and needs help.
The brothers and Bobby get Castiel to the lab and start locating the parts for the doorway ritual. As Sam goes to find the blood, Castiel tells Dean that he regrets what he did to them and says that he’d fix Sam if there was time. Meanwhile, Sam finds the blood and finds Lucifer waiting for him. Lucifer admits that he’s in Hell but that Sam isn’t having a hallucination. He says that Sam never escaped from Hell and that everything that has happened has been a hallucination, torture to make Sam believe that he’s free and then reveal the truth.
As the eclipse approaches, Dean goes to find Sam. He finds the jar of blood but no sign of his brother. With time running out, he goes back and finishes the blood glyph. They position Castiel in front of it and Bobby reads the incantation. As the doorway opens, Castiel tells Dean that he’s sorry. The souls burst from Castiel’s chest and flow back through the doorway into Purgatory. The angel collapses, apparently dead, but after a few seconds his wounds heal. He gets up and thanks them, admitting that he’s ashamed, and promises to redeem himself. Castiel then tells them to run and says that the Leviathans held on and he can’t hold them back. Black veins spread across Castiel’s face and the Leviathans speak through him, telling Dean that they run the show. He throws first Dean and then Bobby across the room and says that it’s going to be fun.
