<-- Previous EpisodeNext Episode -->
Dean wakes up at the hospital emergency room at Sioux Falls General Hospital as the doctors work on him. The attending physician tells him that Sam bashed his head and the doctors are giving him a MRI. When Dean wakes up later, he discovers that he’s in bed with a cast on his broken right leg. He tries to get out of bed and ends up on the floor due to the morphine. Bobby arrives, dressed in a suit, and explains that he got out of his house before the Leviathans burned it. He says that it isn’t safe and tells Dean to meet him at the ambulance while he goes to get Sam.

The Leviathans posing as Dr. Gaines and the head nurse receive a call that the Winchesters are in the hospital.

Dean sneaks out of his room and heads for the ambulance bay. Meanwhile, Bobby finds Sam strapped to a stretcher and takes him to the ambulance. Dean meets them and they drive away just as Gaines and the head nurse come out. Realizing that they’ve lost the brothers for the moment, Gaines calls his boss.

The brothers hole up in Rufus’ old cabin in Whitefish, Montana. Three weeks later, they are close to recovering. Bobby arrives with the Impala and tells them that he’s talked to hunters who have met the Leviathans. They figure that they’re like shapeshifters but eat humans, bleed black goo, and can’t be killed by any known means. Gaines and the nurse have disappeared from the hospital. Sam hesitates when he hears Lucifer’s voice in his head but then snaps out of it and insists that he’s fine. Bobby takes off to recover copies of the books that he lost with his house. Dean asks Sam to make a run for food. Once he leaves, Dean asks Bobby if he thinks Sam is doing okay. Bobby figures that he’s making progress, but Dean worries that the other shoe is going to drop eventually. Until then, Bobby says that they have to deal with the Leviathans.

At the gas station, Sam picks up food and notices a Bozeman newspaper with a headline about an ice pick killer. He plays for it with a credit card and a credit card worker, Chet, picks it up. The worker, a Leviathan, calls Edgar and tells him that they’ve spotted Sam, and Edgar tells him to track the Winchesters down and dispose of them.

Sam returns with the food and Dean asks how he’s handling the hallucinations. His brother insists that he can tell the difference but doesn’t know if he’s getting better. Once Dean dozes off, Sam reads the newspaper article again and remembers…
