Sam and Dean travel to an abandoned sanitarium where they find that the patients once revolted against cruel and unusual punishments given to them by the head doctor. The brothers search the area for lost college students, but the spirits make them go insane, and end up turning Sam against Dean.

At the Roosevelt Asylum in Rockford, Illinois, two police officers notice that kids have broken in again. The older officer, Daniel Gunderson, explains to the new officer, Lt. Walter Kelly, that the abandoned asylum has a bad reputation, and it's rumored that anyone who sleeps there for the night is driven insane. They discover that whoever broken in used heavy-duty bolt cutters to cut the padlock. The officers split up to search and Gunderson's flashlight goes out, while someone watches him from a nearby door. Meanwhile, Gunderson finds the kids and sends them on their way. Kelly meets him outside and says that he didn't find anything. As they get in the car to go, Kelly's nose bleeds briefly...

The Roosevelt Asylum, which is portrayed in this episode, is an actual asylum that is believed to be haunted and has violent spirit activity.
During the filming of this episode,
Smallville star Tom Welling hid in the backseat of the set car. The filming location of
Smallville was very near to where this episode was filmed.
The coordinates that John Winchester texts his sons are 42, -89, which are the coordinates for the asylum in Rockford, Illinois.

While looking at the article about the asylum, the picture of the wendigo is on the right hand page. Later when the look at the article, the wendigo is on a different page than before.
Cultural References
Dean: Let me know if you see any dead people, Haley Joel.
This is reference to the movie The Sixth Sense. In this movie the character Cole Sear, interpreted by the actor Haley Joel Osment, can see dead people, and says the famous line "I see dead people".
Dean: So who do you think is the hottest psychic? Patricia Arquette? Jennifer Love Hewitt? Or you?
Patricia Arquette and Jennifer Love Hewitt have their own "supernatural" tv show,
Medium and
Ghost Whisperer. Their characters are very similar.
Dean: Nigel Tufnel
This alias that Dean gives is the name of the lead singer of the fictional band Spinal Tap. Nigel is played by actor Christopher Guest.
Dean: Don't ask don't tell.
"Don't ask, don't tell" is the United States military policy that began during WWI and is still used today concerning sexual orientation of its recruits.
Dean: All work and no play makes Dr. Ellicott a very dull boy.
This is a reference to Jack, Jack Nicholson's character in The Shining, who is a psychotic caretaker of a hotel. Jack was writing a novel, but all he wrote in the book was "All work and no play makes Jack a very dull boy" over and over.
Dean: Dr. Feelgood was working on some sort of extreme rage therapy.
Dr. Feelgood is a nickname given to doctors who overprescribe psychoactive "feelgood" medicine to their patients.
