Sam and Dean are hunting a Rawhead in an abandoned building and find the two children that it has abducted. As they get the children out of the basement, the creature grabs Sam as he goes up the stairs. Dean fires a taser at it and tells Sam to get the children out, while he goes in after the monster. It knocks him down into a puddle of water on the floor and comes after him, and Dean fires the taser again. It shocks the creature, which is standing in the puddle, but electrocutes Dean as well. Sam returns and finds his brother, dying.
Sam gets Dean to the hosptal and pays for it with one of their stoloen credit cards. The police are there to take a statement, and Sam claims that they heard screams as they were driving by, and went to investigate. The doctor tells Sam that the shock triggered a heart attack, damaging Dean's heart, and they're only giving him a month to live at most. Sam refuse to accept that, but Dean appears resolved to his fate. When Dean says there's nothing that Sam can do to save him, Sam insists otherwise. He goes to the motel and spends three days doing research and tries to call John without success. Dean checks himself out and returns to the room, but Sam figure he's putting up a cheery face. He explains that one of John's friends, Joshua, has referred them to a specialist in Ford City, Nebraska.
They arrive at the "specialist" and Dean is disgusted to see that Sam has taken them to see a preacher, a faith healer named Roy Le Grange. As they go in, a protester, David Wright, complains that Roy is bilking his audience, but the police move him along. Sam figures that there must be something to Le Grange's claims, but Dean insists that he only believes in what he can see and touch. A young woman, Layla Rourke, overhears them talking and insists that Dean has to have faith. They talk briefly before Layla's mother takes her away.
The brothers go into the revival tent and note the security cameras. Sam gets Dean to sit up front over his brother's objections, and Ray gets up to address the crowd. Ray, who is blind, talks about God rewarding the good and punishing the corrupt. Sam notes that Ray has a Coptic cross among his decorations. Meanwhile, Ray says that God picks out who to heal. When Dean complains that Ray's a con artist under his breath, Ray hears him and calls him up. Dean balks, but the audience and Sam cheers him on. Ray asks everyone to pray, and then places his hand on Dean's head and calls upon God to heal him. Dean winces and then sinks to his knee, and finally passes out. Sam rushes to his brother's side while the audience applauds. Dean wakes up and looks up, and sees a brief image of an old man in a black suit at Ray's side.
Later, Sam takes Dean to see a doctor, who says that there's no sign of heart trouble. She comments that it's ironic, since a local athlete, Marshall Hall, died of a heart attack the previous day. Once she leaves, Dean figure that something is going on, and explains that he felt cold when he was being healed, and saw the old man. He figures the old man was a spirit, and tells Sam that he's going to have to trust his instinct on it.
Dean goes to see Ray and his wife, Sue Anne. She insists that her husband performs miracles regularly. Ray explains that he woke up one day, blind due to cancer, and the doctors gave him a month. He prayed for a miracle, went into a coma, and woke up without the cancer. After that, Ray discovered he couild heal people. Dean asks him why Ray chose Dean, and Ray insists that the Lord guided him, and Dean stood out from all the rest as a young man with an important job that isn't done.
Sam talks to Marshall's friend, Jason, who says that his friend claimed he saw a ghostly figure before he died. As he leaves, Sam notices that the wall clock has stopped, and Jason confirms that it stopped at the moment that Marshall died.
As Dean leaves Ray's house, he runs into Layla, who explains that he rmother is there to talk to Ray. Sue Anne tells them that Ray is resting, and he can't help Layla until the Lord allows. Mrs. Rourke glares at Dean and wonders why he's still there. When Layla tries to stop her, Mrs. Rourke says that they've been at every prayer meeting but Ray hasn't healed her daughter and has helped non-believers like Dean instead. Layla finally explains that she has an inoperable brain tumor, and assures Dean that it's okay. Her mother doesn't think so, and asks Dean why he deserves to live more than her daughter.
Back at the motel, Sam apologizes and then explains that Marshall died at the exact moment that Dean was healed. He's put together a list of the six people that Ray has healed, and each time someone local died of the same thing. Dean insists that Sam shouldn't have brought him there, and now someone is dead because of him. Sam wonders how Ray is trading a life for a life, but Dean figures that the old man he saw is the one responsible.
Ray is conducting another revival meeting, and curring a man of emphysema.
In the woods, an abortion advocate is jogging through the woods when she senses someone nearby. She stops to catch her breath,and finds the old man in the suit facing her.
Dean realizes that they're dealing with a Reaper.
The old man pursues the jogger through the woods and easily catches her. He touches her and she dies. The Reaper, invisible to the crowd, touches the dying man and heals him.
As they check the Internet, Dean explains that there are multiple Reapers, and that they can only be seen by their victims. Sam remembers the Coptic cross at the church tent and confirms that it's the cross on the Tarot Death card. They figure that Ray is using black magic to bind the Reaper, but it's extremely dangerous. Dean says that they have to kill Ray, but Sam refuses to kill a human being. He suggests that they figure out how to break the binding spell.
The brothers go back to Ray's revival camp and Dean says he'll stall Ray while Sam tries to find the spell book that Ray is using. David Wright is handing out pamphlets protesting Ray's work, and the brothers congratulate him. Sam then breaks into Ray's house and finds a small spell book hidden in the shelves. There's a picture of a cross, the same one that is in the tent. There are also articles on an openly gay teacher who won a lawsuit, an abortions advocate, and Wright's protests against what he claims is a cult. Sam calls Dean and says that Ray is choosing victims that he believes are immoral, and figures Wright is the next victim. He tells Dean to stop Ray from healing anyone.
Ray begins his service and calls Layla up to be healed. As she goes up, Dean warns her that she can't let Ray heal her. She doesn't undertand, and all he can say is that somethign bad will happen. Layla decides to go up, and Ray takes her hand and asks everyone to pray.
In the parking lot outside, Sam hears Wright screaming. Wright can see the Reaper approaching, but Sam can't. He tries to get Wright away.
In the revival tent, Dean yells that there's a fire. As the people go out, Mrs. Rourke begs Ray to heal her daughter. Dean calls Sam, but the Reaper is still coming for Wright. They figure that Ray isn't controlling it. Dean sees Sue Anne at the back of the tent and grabs her arm. She's holding a smaller version of the Coptic cross, and yells for help. The police take Dean away, while outside, the Reaper fades away before Wright dies.
Outside, Sue Anne asks why Dean is interfering after everything Ray has done for him. She says that she won't press charges, and the Lord will deal with him. Layla asks why Dean stopped her healing, and he tries to tell her that Ray isn't the answer. She tells him goodbye and walks away, while Dean figures she deserves a healing more than him. As he goes, Dean overhears Ray saying that he'll hold a private healing for Layla that night.
At the motel, Dean explains that Ray has no idea what Sue Anne is doing. Sam shows Dean the book he found and the binding spell that it contains. It requires a black altar, and Dean figures that she used the spell to save Ray and cheat Death. Now she's using the Reaper to kill off people she believes are immoral, and bolster her husband's reputation as a healer. Dean figures that they have to get the cross and destroy the altar, and they have to do it soon.
At the revival, San and Dean arrive and spot Layla's car. Dean wonders if they have the right to stop Layla from being healed, but Sam says that they can't let someone else die to save her. They go to the tent and see Ray holding a private healing for Layla. There's no sign of Sue Anne, and Dean tells Sam to find her while he draws off the deputies watching the house. Sam runs to the house and sees a light through the cellar door. He goes down and finds the black altar. There's a photo of Dean, his face crossed off, and the Coptic cross. Sue Anne comes in and says that she can give Dean his life and take it away. As Sam overturns the altar, Sue Anne runs upstairs and bars the door. She insists that God gave her the power to punish the wicked, and Den deeserves to die so Layla can live.
As Ray prays over Layla, the lights near Dean go out. He turns and sees the Reaper approaching him. He grabs Dean as Ray touches Layla, while Sue Anne prays outside the tent and holds the Coptic cross. Sam gets out of the cellar, grabs the Coptic cross, and shatters it on the ground. The Reaper goes to find Sue Anne and smiles in triumph. It touches her and she dies of a stroke. Inside, Layla realizes that nothing has happened, and Ray calls out to his wife.
Back at the motel, Dean wonders if they've done the right thing, and admits that it doesn't feel like it. Layla comes to see them, and she explains that Sam called, claiming that Dean wanted to say goodbye. Sam goes out to leave them alone, and Layla explains that Ray didn't heal her. When Dean talks about how it must be tough to believe in the face of that kind of loss, Layla says that she's okay with it, and that it's necessary to have faith even where there are no miracles. As she goes, Dean says that he's going to pray for her, and Layla says that's a miracle.
