When Sam and Dean find articles about a town where children are inexplicably falling into comas, the brothers discover that a Shtriga, a creature that attacks children, is behind the mysterious comas. During the battle, Dean remembers a past battle with the creature that almost resulted in Sam's death.

Sam: An old person, huh?
Dean: Yeah.
Sam: In the hospital? Woo! Better call the Coast Guard!
Dean: Well, listen, smartass. She had an inverted cross hanging on her wall.
 
Patient: Who the hell are you?! Who's there?! Ya trying to steal my stuff?! They're always stealin' around here!
Sam: No, uhh, ma'am, we're Maintenance. We're sorry, uhh, we thought you were sleepin'.
Patient: Ahh, nonsense! I was sleepin' with my peepers open! (laughs maniacally) Fix that crucifix, would ya? I've asked four damned times already!
 
 
Episode Goofs
 
While driving to Fitchburg, Sam and Dean drive by a sign that reads "Fitchberg", which is directly in front of a sign that says "Welcome to Fitchburg!" The camera freezes on the two of them simultaneously, making it blatantly obvious.
 
When Dean and Sam are supposed to be hiding around the corner in the hospital, and the doctor walks past, seemingly not noticing them, you can his eyes slightly divert to the left, towards them.
 
 
Cultural References
 
Brockway, Ogdenville, and North Haverbrook

These cities are the cities in which similar deaths had occurred, as found in the newspaper clippings by Sam. In The Simpsons episode "Marge vs. the Monorail", Brockway, Ogdenville, and North Haverbrook are the cities that monorails were sold to.
 
ThunderCats

In the flashback to Sam and Dean's childhood, Sam is seen watching ThunderCats, a 1985 cartoon about cat-like creatures who battle evil on a foreign planet.
 
Episode Title: Something Wicked

The episode title has two possible references: one to Something Wicked this Way Comes, a 1962 novel by Ray Bradbury that deals with the supernatural; and one to the line "By the pricking of my thumbs, something wicked this way comes" from William Shakespeare's MacBeth.
 
 
Analysis
 
Shtriga

A creature of Albanian folklore, Shtrigas are creatures that often target young children and drink their blood. Those who were attacked by the Shtrigas often died since only the Shtrigas themselves could cure the infected. Though Sam and Dean are able to defeat the creature, infected children, in folklore, were only cured if the Shtrigas spit in their mouths.
