Episode Notes
Instead of the thunderstorm-themed opening credits from season 1, fire-themed credits are now used. Also, the "A" in Supernatural is pentagram-shaped.
John Winchester dies at 10:41 AM.
Unused scenes include:
1) Episode 501, Scene 11: Dean ogles a girl who is stripping for an exam, and starts to go into her room for a better look. A dying man on a gurney grabs Dean and begs him not to let him die. When Dean turns away for a moment, the man disappears.
2) Episode 501, Scene 23A: Right before Dean hears Tessa crying for help, he sees the same bloody man in a hospital bed, begging for help. A patient and his visitor are at the adjacent bed and don't react. The bloody man disappears a second later.

Dean: Come on, Dad, you gotta help me. I gotta get better, I gotta get back in there. You haven't called a soul for help. You haven't even tried. Aren't you gonna do anything? Aren't you even gonna say anything? I've done everything you've ever asked me. Everything. I've given everything I've ever had. Are you just gonna sit there and you gonna watch me die? What the hell kind of father are you?
Tessa: What’s happening to me? Am I dead?
Dean: That sorta depends.
(looking at Tessa's comatose body)
Tessa: I don't understand. I just came in for an appendectomy.
Dean: I hate to bear bad news, but I think there were some complications.
Dean: Have you heard of an out-of-body experience?
Tessa: What are you, some New Agey guy?
Dean: You see me messing with crystals or listening to Yanni?
Sam: Hey, I think maybe you're around and if you are... you'll make fun of me for this but, umm... there's one way we can talk. (pulls out a Ouija board)
Dean: Oh, you gotta be kidding me!
Sam: (sets up on the floor) Dean? Dean, are you here?
Dean: God, I feel like I'm in a slumber party.
Tessa: It's an honorable death. A warrior's death.
Dean: I think I'll pass on the 72 virgins, thanks. I'm not that into prude chicks anyway.
Tessa: (deadpan) That's funny. You're very cute.
Tessa: Well, like you said, there's always a choice. I can't make you come with me, but you're not getting back in your body and that's just facts. So yes, you can stay, you'll stay here for years disembodied, scared, and over the decades it'll probably drive you mad. Maybe you'll even get violent.
Dean: What are you saying?
Tessa: Dean, how do you think angry spirits are born? They can't let go, they can't move on, and you're about to become one. The very thing you hunt.
Tessa: It's time to put the past behind you.
Dean: And go where?
Tessa: Sorry. I can't give away the big punchline.
John Winchester: You know when ... when you were a kid, I'd come home from a hunt, and after what I'd seen I'd be... I'd be wrecked. And you, you'd come up to me and you put your hand on my shoulder, you'd look me in the eye and... you'd say "it's okay, Dad." Dean... I'm sorry.
Dean: What?
John Winchester: You shouldn't have had to say that to me. I should have been saying that to you. You know, I put... I put too much on your shoulders, I made you grow up too fast. You took care of Sammy, you took care of me. You did that, and you didn't complain, not once. I just want you to know... that I am so proud of you.

Episode Title: In My Time of Dying
"In My Time of Dying" is a song by Led Zeppelin.
Dean: Dude, I full on Swayzed that mother!
This is in reference to the 1990 movie, Ghost, starring Patrick Swayze and Demi Moore. Swayze's character is killed and remains with his lover as a ghost as he seeks out a way of communicating with her.
Hospital Loudspeaker: Paging Dr. Kripke.
"Kripke" is a reference to the series' creator and executive producer, Eric Kripke.
Dean: Gimme some Ghost Whispering or something!
Dean's comment refers to another currently airing show with a vastly different take on the supernature:
Ghost Whisperer. In that show, the lead character can speak to the dead, who are generally just misunderstood (although at the end of the last season some darker elements have been added). Dean hopes his brother's erratic psychic abilities might help them communicate in a similar way here.
