The episode picks up where last season's cliffhanger episode left off. Dean is in the hospital in a coma following being rammed into by the semi truck and doctors do not give him much chance of surviving. Yet he is able to walk the halls as a disembodied spirit. There he meets Tessa, a young woman apparently in a similar predicament, and sees a strange apparition that seems to draw the life from the sickest patients. Unable to save Dean any other way, John Winchester attempts a very dangerous plan to save his son's life.

Episode Quotes |
| |
Dean: (to Sam) Come on, you're the psychic! Gimme some ghost whisperin' or something! | John: Here, give 'em my insurance.
Sam: "Elroy McGillicuddy"?
John: And his two lovely sons. | John: Where's the Colt?
Sam: Your son is dying, and you're worried about the Colt? | Dean: I think I'll pass on the 72 virgins. I'm not that into prude chicks anyway. | Sam: Hey, I think maybe you're around and if you are... you'll make fun of me for this but, umm... there's one way we can talk. (pulls out a Ouija board)
Dean: Oh, you gotta be kidding me!
Sam: (sets up on the floor) Dean? Dean, are you here?
Dean: God, I feel like I'm in a slumber party. | Tessa: It's time to put the past behind you.
Dean: And go where?
Tessa: Sorry, can't give away the big punchline. | Tessa: What’s happening to me? Am I dead?
Dean: That sorta depends. |

Episode Goofs |
| |
During the accident, the demon driver rips the door and mirror off of the car. However, when Sam and Bobby go to the junkyard to find the car, the mirror is still attached to the car. | As the doctors perform CPR on John, they push on his chest right below the ribs. In the proper administration of CPR, one presses right over the heart, not below the ribcage. |

Episode Title: In My Time of Dying
"In My Time of Dying" is a song by Led Zeppelin. | Dean: Dude, I full on Swayzed that mother!
This is in reference to the 1990 movie, Ghost, starring Patrick Swayze and Demi Moore. Swayze's character is killed and remains with his lover as a ghost as he seeks out a way of communicating with her. | Hospital Loudspeaker: Paging Dr. Kripke.
"Kripke" is a reference to the series' creator and executive producer, Eric Kripke. | Dean: Gimme some Ghost Whispering or something!
Dean's comment refers to another currently airing show with a vastly different take on the supernature: Ghost Whisperer. In that show, the lead character can speak to the dead, who are generally just misunderstood (although at the end of the last season some darker elements have been added). Dean hopes his brother's erratic psychic abilities might help them communicate in a similar way here. |
