Bela has made a delivery and is returning to her car when a man confronts her at gunpoint: Gordon Walker the rogue Hunter. Bela has heard of him by reputation and thought he was in prison, but Gordon notes he escaped. He demands to know where Sam is, claiming he’s the Antichrist. Bela refuses to tell him... unless he pays her upfront. He doesn’t have any cash but she takes his hundred-year-old mojo bag then calls Dean.
Sam and Dean enter a building on the trail of a killer and find her victim: a man bleeding from the throat. Dean goes after the killer and cuts his arm to draw her out. The woman is a vampire, and she grabs Dean and starts to bite his neck. He injects her with dead man’s blood, paralyzing her. Sam shows up and worries that Dean let her get too close for comfort, but his older brother laughs it off. They interrogate the vampire, Lucy, and she has no idea that she’s a vampire. She says she was picking up a new drug in a bar and thinks the entire thing is a hallucination. There’s no cure and Dean has to decapitate her to keep her from killing again.
Gordon and his ally Kubrick go to the hospital to interrogate the victim, pretending to be CDC agents. Fortunately, he didn’t get any of the vampire’s blood in his body, so they don’t have to kill him to prevent the transformation. The victim thinks they’re joking.
Sam and Dean go to the bar and spot the drug dealer giving another woman the vampiric blood/drug. He runs away and they follow but they draw up short when Gordon and Kubrick arrive, having followed the trail. The two men open fire and Dean charges off, drawing their fire, while Sam heads for their hotel. Kubrick and Gordon split up but Dean loses Kubrick and the dealer, Dixon, attacks Gordon.
Dean eventually gets back to the hotel and Sam is again angry that he casually risked his life. They figure out Bela set them up and Dean calls her, and she admits she gave Gordon the info. Dean coldly informs her that he’ll kill her the next time he sees her.
Gordon wakes up to find himself in a warehouse with two women, chained to the ceiling. Dixon comes in and feeds the women blood, then tells Gordon that he knows who the hunter is and how many vampire he’s killed. He plans to let the transformed women feed on Gordon for revenge, but when Gordon is unrepentant, Dixon has a better idea and mingles his blood with the hunter’s.
Dean and Sam try to figure out where Gordon is and Sam coldly states that they’ll have to kill him. Dean is surprised but before he can follow up, they get a call from Bela. She used an Ouija board to locate Gordon but warns that the spirits are saying that Dean and Sam should get out of town. They ignore the warning and head out.
Gordon wake up chained to the ceiling like the two women, and squints into the dim light. He rips the chains out of the ceiling and the women ask him for help. He considers them... and a few minutes later walks out. With his heightened vampire senses he’s drawn to the heart beat of a man fixing his flat tire. Gordon attacks the man in his car, drinking the blood.
Dean and Sam get to the warehouse and find the two women... who have been decapitated. Dixon is also there, and admits that he messed up by making Gordon a vampire.
Gordon goes to Kubrick’s rental and reveals that he’s a vampire. He wants Kubrick to kill him, but not until after Gordon kills Sam with his newfound abilities. Kubrick doesn’t trust him and tries to kill him, but Gordon casually thwarts the attack and kills him in turn.
Sam and Dean check all the hotels and rentals where Gordon might be staying, but don’t turn up anything. They buy new cell phones and destroy the old ones to prevent Gordon from tracking them from Bela’s call, then cover their scent and hole up. Gordon calls them on their new phones, having used his heightened scent to find out where they made the purchase. He tells them that he has taken a women captive and if the Winchesters don’t come, he’ll kill her.
The guys arrive and free the women, but a metal door slams down, cutting off Sam from Dean and the women. Gordon switches off the lights and stalks Sam, using his heightened vision to follow him in the dark while taunting him. He says neither one of them is human and that he’ll kill Sam and then himself. When Sam accuses him of being a monster, Gordon agrees, admitting he made the captive a vampire. Outside, the woman attacks Dean and he’s forced to use the Colt to destroy her.
Gordon finally goes after Sam, knocking him through a wall and into the next room. Dean shows up and tries to shoot him, but Gordon knocks him out. That gives Sam enough time to grab a piece of barbed wire and wrap it around Gordon’s neck, then pull hard enough to decapitate him.
Later on the road, Dean is working on the Impala but then calls Sam over and starts showing him what to do. Sam is surprised Dean is even letting him touch the car, and Dean admits that it’s time he face the future and make sure Sam is ready for when his older brother is gone.
