Dean is running through the woods and finds himself face to face with a hellhound. It follows him as he tries to run away, and brings him down. As it starts to rip him open… he awakes, having dozed off while reading up on hell hounds. Sam comes in and admits the only have 30 hours left before Dean's year-long grace period ends and Lilith can collect his soul. Bobby has a lead on Lilith but as they leave, Dean hallucinates that Dean's face is corrupted.

Bobby performs a ritual using Lilith's name that lets them locate the demon in New Harmony, Indiana. However, Dean refuses to go, insisting that there's no hope for him and he doesn’t want Sam killed trying to save him. Sam says they should try and get the demon-killing knife from Ruby but Dean refuses to hear him out. Bobby goes off to find something else while Sam goes to the basement and performs a ritual to summon Ruby. She admits that she knew about Lilith but didn't tell them because she doesn't think they're ready to take the uber-demon on. She also notes that Lilith is "relaxing" in New Harmony and it's the perfect time to strike. However, they need something stronger than the knife: Sam's psychic abilities. He insists that he doesn't have them any more but Ruby claims that it's only now that he's desperate enough to save his brother that he can tap into them.

Dean has figured that Sam would try and summon Ruby and comes down stairs. She tries to convince him she's telling the truth but he doesn't believe her and punches her. She fights back starts knocking him and Sam around. However, Dean manages to get the knife from he and when she come for it, finds herself in a demon's trap that he set earlier. They leave her to rot while they take the knife and pack for the trip to New Harmony. Sam wonders if Lilith is right that his powers are their only chance, but Dean says they need to fight demons the way they always have. He admits that they have to stop making the same mistakes: every time one of them gets into trouble, the other one endangers himself to save his brother.

In New Harmony, a grandfather, Pat Fremont, comes out to check his mail and chats with his neighbor Tom. As Pat leaves, he slips a note to Tom sailing "Help me." Back in his son's house, he steps over a dead and rotting corpse of the babysitter then talks to his son and daughter-in-law. They're all terrified of Pat's granddaughter, a little girl who comes in covered with blood and talks about how she killed the cat for scratching her. Her father tires to get her to let them go but she refuses and wonder if they love her: they hastily assure her they do.

Dean and Sam try to get the Impala started but discover that Bobby took the distributor cap. He insists that he's going with them, and has figured that dean is suffering from hallucinations as the hell hounds get closer. He follows them toward New Harmony, as the brothers end up bonding over Bon Jovi on the tape deck. A policeman pulls them over for a traffic violation, but Dean suddenly attacks him and kills him with the knife. He says that he can see demons now despite their human shells, and Bobby figures that the closer he gets to Hell, the more he can see of other individuals from Hell.

The Fremonts are having birthday cake for yet another night and the daughter tells Pat that he was being bad by trying to contact his neighbor. She telekinetically kills him and the parents start eating cake. Sam, Dean, and Bobby are watching and Sam prepares to intervene, but Dean warns that he's spotted demons all over the neighborhood. However, they have to stop the girl before she kills again.

Mrs. Fremont reads her "daughter" a bedtime story while Dean and Sam start killing the neighborhood demons and Bobby prepares a bucket of holy water. Ruby arrives, somehow managing to have broke out of the demon's circle, and grabs Dean. Sam has the knife and threatens to use it on her, but they're interrupted when the neighborhood demons spot them. They run for the Fremont home and try to get the locked door open. The demons close in but Bobby has dumped the holy water into the lawn sprinkler and turns it on, driving the hellspawn off.

The others get inside and Mr. Fremont begs them for help. He tells them his daughter is upstairs but when he tries to go with them Dean knocks him out and takes him to the basement while Sam and Ruby go upstairs. They find Mrs. Fremont and her daughter in bed, the daughter asleep. Mrs. Fremont whispers to Sam to kill her daughter and he stabs downward, but Dean arrives and stops him just in time . His newfound vision confirms she's no longer possessed.

The demons wait outside while bobby watches from hiding. With only seconds left, Sam asks Ruby what he needs to do. However, Dean refuses to let him sacrifice himself and ruby warns it takes time for Sam to learn how to use his psychic gifts. The invisible hell hounds arrive at midnight and the trio barricade themselves in a room with mystic dust to keep out the hounds. Ruby asks for the knife so she can fight the hell hounds. However, Dean realizes with his vision that Lilith has possessed her. The uber-demon telekinetically pins both of them and just laughs when Sam tries to bargain with her. Dean demands to know what she wants but she opens the doors and let the hell hounds start ripping at Dean. She then turns to Sam and unleashes a bolt of glowing white energy. When the light fades… he's unaffected. He grabs the knife and kills her, but it's too late for Dean who is dead on the floor. Sam holds is brother in his arms and cries.

Dean is suspended by barbed wire in an endless void, and screams in agony with the other souls of the damned. All he can do is call out for Sam.
