Johnny and Sarah are wrapping up a date with a late meal at a drive-in. Sarah’s concerned about her kids, and admits it’s the first time she’s been away from Hope since her baby daughter was born. Johnny tries to reassure her and they almost end up brushing lips for a moment, but quickly dismiss the moment.

They’re driving back to Johnny’s place when Johnny starts having stomach pains. Sarah takes him to a nearby hospital, a rural clinic, where the nurse, Carrie, reassures her that Johnny is in good hands with Dr. Cole. Cole diagnosis Johnny as having appendicitis and has him taken into surgery. As Johnny goes, he starts mumbling cryptic phrases about lions and flowers and “freezing in Reno,” and the fact that Cole is going to try and kill him so she should get Sheriff Turner.

Later Sarah calls home to make sure the kids are okay, then spots Cole wheeling a bald man in a chair. She talks to Cole who tells her that Johnny came through surgery fine. She sits at his bedside but he doesn’t wake up: Johnny’s gone back into a coma.

Cole warns her that due to Johnny’s previous accident, he’s suffering from a coma and may not revive. Sarah kisses Johnny out of regret for what they’re missing. She confronts Cole with some treatment suggestions but he prefers to wait and watch, and suggests she get a second opinion. Sarah can’t get through to Johnny’s previous doctor, but overhears an orderly playing piano for three patients in similar vegetative states. The man, Dewey Morris, mentions that of the three patients two of them have no one and the other one, Marcy, has a brother who only visited her once and then left for good. As the nurse takes them out, she mentions that Marcy’s name is lions and the other woman’s name is Flowers.

Realizing Johnny’s words were part of his vision, Sarah tries to figure out what he meant. She talks with Carrie and figures out the nurse is dating Cole, and Carrie can’t figure out why an attractive single guy would be involved with her. Sarah then bumps into the bald patient Cole was moving earlier. The man says that Cole is trying to kill him and asks Sarah for help. She turns for a moment and he seemingly slips away without a trace.

Sarah decides to call Sheriff Turner, despite the fact she’s running the investigation that is discrediting Walt. Turner looks into Cole’s background while Sarah spots Cole going into a wing of the hospital under renovation. She sees a partially obscured sign just saying “Reno” and realizes this is more of Johnny’s vision. She follows him into a new kitchen area and opens a freezer compartment to find the bald man covered with a sheet. The man grabs her hand briefly before expiring, and Sarah goes to get Turner. Turner report that Cole is a well-respected family man and the background check turned up nothing. She goes with Sarah to the freezer compartment but there’s no sign of the man Sarah saw. They go to Dewey, who recognizes the man’s description as Marcy Lyon’s brother who seemingly left her there alone. They then go to Carrie, telling her that Cole is a married man who has been hiding it from her. Carrie confesses that Cole is running an insurance scam, taking in comatose patients with no families and then forging power of attorney so he can drain their bank accounts while keeping them in comas.

Sarah figures that Cole had to dispose of Mr. Lyons when he came to get his sister, and the Doctor put Johnny into a coma when he overheard his feverish words fro his vision, and will finish him off. She goes to confront Cole at Johnny’s bedside, but he starts to strangle her…

… and Johnny wakes up from what has been his vision of the entire thing, from the moment that Sarah kissed him. He fakes being in a coma when Cole comes in, then he and Sarah go to get Dewey to help them find Marci’s brother Mr. Lyons. They get to him in the freezer compartment and Dewey revives him, while Cole comes after them. After a brief struggle, Sarah knocks him out with a bedpan.

Turner hauls Cole to jail after Johnny tries to explain his vision to her and the fact it was scrambled by the fever. Johnny has to tell her that Sarah went to and got help from Turner in his future-vision, despite her animosity toward the sheriff. The other patients are brought out of their comas and thank Sarah and Johnny for their help. As Sarah takes Johnny home, he points out that she did kiss him and she admits that after getting back to the life she first planned for, with her and Johnny together, she couldn’t stand to lose him to a coma again. After a few moments of silence they kiss for real.
