Tony Soprano leads two lives, one where he is a good family man to his wife, two children, and his widowed mother and the other as part of the New Jersey mafia. He soon begins suffering from panic attacks and he begins seeing a female psychiatrist, Dr. Jennifer Melfi, which he knows if members of the "family" find out, he will be killed. Meanwhile, Tony's Uncle Junior wants to use Tony's childhood friend, Artie's, restaurant to whack a guy by the name of Pussy Malenga.
A truck hijacking goes wrong for Christopher and Brendan and it angers Uncle Junior. Tony has to force his mother into a retirement home, something she doesn't let happen easily. Meanwhile, Giacomo "Jackie" Aprile, who is the acting boss of the family, is deathly ill with cancer.
Junior is upset that Comley thanks Tony for returning the stolen goods to Comley Trucking, so he goes to visit Livia at Green Grove. However, Tony's mind is on Jackie Aprile who is in the hospital now and also feels guilty about Artie and Charmaine Bucco who are having to undergo a second arson investigation for the Vesuvio fire. Meanwhile, Meadow asks Christopher and Brendan for some help in studying for their SATs.
Tony keeps having strange nightmares and some involve a naked Dr. Melfi, so he decides to have her followed. Christopher continues to worry even after he leaves the emergency room about who really beat him up. Tony decides that he is going to resolve all his issues with Uncle Junior. Meanwhile, after Anthony, Jr. continues to get into fights at school, Meadow decides to complicate things for him by finally revealing what their dad really does for a living.
Tony decides to escort Meadow to Maine where she is going to look at prospective colleges. While there, he notices an old "friend" and his trip quickly changes from helping his daughter along her educational way, to just helping her survive the weekend. Meanwhile, Carmela, who is back home and suffering from the flu, gets a surprising visit from Father Phil and a even bigger surprise phone call from Dr. Melfi.
Junior is more than thrilled when he is named the new Boss of New Jersey and no one is more upset than Tony. After Junior decides to tax Hesh, Tony goes behind his uncle's back and sets up a meeting between Junior, Hesh and Johnny Sack, so the later two can get Junior to change is mind. Tony and Carmela end up celebrating their 18th wedding anniversary in Johnny's Manhattan restaurant where Carmela realizes that she is jealous of Dr. Melfi more than she ever has been with one of his mistresses. Meanwhile, Tony continues to have erotic dreams of Melfi and even tries to kiss her at the end of their session.
Anthony Jr. ends up acting out at school and Tony has no idea what to do about it. However, the school psychologist thinks that A.J. may have Attention Deficit Disorder. All of A.J.'s problems make him remember his childhood, which Dr. Melfi insists does not mean that A.J. will end up the same as him. Meanwhile, Livia discovers, through A.J. that Tony is seeing a psychiatrist.
The talk of the town is the indictments that are scheduled to come down and how Tony and his associates are going to deal with them. Christopher later becomes upset because when the associates were named on television, he was not one of them. Meanwhile, Livia decides it's the perfect time to tell Junior that Tony is seeing a psychiatrist.
Junior decides to head to Boca Raton to visit his lady love, Roberta before the indictments come down. Meadow decides to tell Tony and Carmela that her soccer coach had sex with one of her friends. Meanwhile, Tony learns of a secret that Junior would rather he, nor anyone else, know.
Carmela and Dr. Melfi agree that Tony needs to expand his horizons and meet new people, so he and Carmela decide to begin socializing with the neighbors. Meanwhile, Christopher and Adriana decide to try to make it in the music business after a chance encounter with a rap star by the name of Massive Genius.
Tony discovers that there is rat in his New Jersey family and rumor has it the perpetrator is none other than Pussy Bonpensiero. Meanwhile, Junior puts a hit out on Tony.
Tony has become severely depressed since Pussy has mysteriously disappeared. Dr. Melfi has increased his Prozac dosage and placed him on Lithium and he begins to think about a beautiful Italian woman named Isabella. Junior finally makes his move and Tony is attacked while stopping to buy some juice at a newsstand. After the attack fails, Junior and Livia await their fate as an FBI agent tries to get Tony to say he was the target of a hit so he and his family can go into the Witness Protection Program and give up the rest of the New Jersey mob.
Tony and Junior decide what to do with Jimmy Altieri after they both agree he is wearing a wire. The FBI allows Tony to listen to a tape from Livia's room at Green Grove and he discovers just who wanted the hit to be made on him. He also discovers that the FBI, his mother and his uncle all know he is seeing Dr. Melfi. This makes Tony fear for Melfi's life and he informs her that she needs to get out of town. Meanwhile, Livia has a stroke after a run-in with Tony.
Tony assumes the role of boss of the family after Junior is indicted on 12 RICO counts. Christopher begins selling phony stocks to senior citizens after getting a brokerage license from the SEC by paying someone else to take the test for him. Pussy is discovered alive and has a meeting with Tony. Meanwhile, Livia is still recovering from her stroke which brings Tony's older sister Janice in from Seattle and Carmela wants Tony to go back to therapy, the only problem is that Dr. Melfi does not want anything to do with him and will not refer him to another psychiatrist.
Pussy gets a visit from FBI agent Skip Lipari and ends up becoming a rat for the Fed's case against Tony. Livia is convinced that Tony and Janice are planning on getting rid of her -- literally. Meanwhile, Junior gets out of jail on a medical release.
Jackie Aprile's older brother Richie returns to Jersey after being in prison for 10 years. However, Richie immediately dislikes the way that Tony is running the family and decides to have a chat with Junior. Meanwhile, Meadow throws a party a Livia's house and comes home drunk. Tony and Carmela disagree about how to punish her. Also, Dr. Melfi makes a visit to her psychiatrist Dr. Elliott Kupferberg.
Tony and the guys decide to make a visit to Italy as Carmela stays back at home and contemplates their marriage. While there, Tony finds himself attracted to Annalisa who happens to be the daughter of another mob boss. Angie Bompenserio tells Carmella that she wants to divorce Pussy.
Furio Giunta, the newest member of Tony's crew, has arrived from Italy and thanks to Artie Bucco is now the head mozzarella maker at the Nuovo Vesuvio Restaurant. All this upsets Christopher and Pussy. As time goes on, Tony becomes angrier than usual especially after finding out Janice and Richie, who used to be lovers, have reunited. Just as Tony is about to lose it, Dr. Melfi calls him up and asks him if he would like to resume his therapy sessions.
Tony decides to lie to Dr. Melfi and tells her that everything in life is fine, but she knows better. However, he does admit that he does a have a slight problem with "happy wanderers" who haven't a care in the world. He soon has to organize the "Executive Game", a high stakes poker game that his father and Uncle Junior started over 30 years ago. Things go awry when Richie shows up unannounced. Meanwhile, Meadow is upset after Tony gives her a Jeep that he took in the poker game and Janice tells Richie that he has to start standing up to Tony or he will come to regret it.
After A.J. crashes his mother's car and begins denouncing God and the Catholic Church, Tony has about had it. However, Dr. Melfi tells him that it's natural for him to act this way at his age and that he has finally discovered that he is the one responsibly for all his actions. Pussy has been asked by the Feds to wear a wire to A.J.'s confirmation Meanwhile, Christopher once again has been slacking in his responsibilities and Tony is more than upset when he finds out he has been hanging out with a filmmaker instead and he asks him to make a choice immediately.
Carmela is concerned that Meadow might not get into a good college despite her wonderful grades and SAT scores and asks the twin sister of her neighbor Jean Cusamano to write Meadow a letter of recommendation since she is an alum of Georgetown Law School. Richie decides to give Tony a leather jacket that means a lot to him in the hopes he can get in good with the boss. Meanwhile, Christopher gets caught in the crossfire outside a diner.
Christopher survives the attempt on his life, although he tells Tony and Paulie that he almost died and saw Hell. Carmela informs Tony that she has had more than enough of him coming home with his shirts smelling like of perfume and she wants him to end it with his goomah. Meanwhile, Dr. Melfi is finding it increasingly difficult to cope with her disgust for Tony's "profession."
A witness knows that Tony was there when Matt Bevilaqua was whacked, which makes Tony realize if he does not find the person, he will have to leave New Jersey. Tony and Richie Aprile work together to collect Davey's gambling debt. Pussy is contacted by Agent Lipari about Bevilaqua's death. Carmela becomes close to her new wallpaper guy, who happens to be Davey's brother-in-law.
Tony gets warned that the Feds are still planning to come after him even though he's been cleared for the Bevilaqua death. Dr. Melfi takes to drinking vodka before each of her sessions with Tony. Richie and Junior decide to use their garbage routes to sell cocaine. Meanwhile, Junior is more than fed up with his house arrest and tries to contact his ex-girlfriend Roberta Sanfilippo.
Tony tries to break off his ties with Richie and Irina and even goes so far as to ask Dr. Melfi to find a therapist for Irina. Richie decides it's time to make his move on Tony. Meanwhile, Janice has had more than enough of Richie's ways.
Tony is more than thrilled that both Janice and Richie are out of his hair and plots to soon get Livia out of the way as well. After a plate of bad food, Tony begins having hallucinations of sex with Dr. Melfi and one of Pussy as a fish, which convinces him that Pussy has gone to the Feds.
The Feds are still trying to get their RICO case against Tony, but have trouble since Pussy is sleeping with the fishes. Therefore, they get a bug for the Sopranos basement. Meanwhile, Patsy Parisi, twin brother of the deceased Philly Parisi, possibly knows how his brother died.
Tony no longer has to worry about Livia ratting him out to the Feds. Janice returns to town. Meadow's new boyfriend from college causes Tony to have a panic attack.
Christopher discovers that being made is not what he thought it would be like. Janice moves into her mother's house as she tries to get her mother's prized record collection from Svetlana. Dr. Melfi tries to find out why meat causes Tony to have his panic attacks. Meadow is upset about Tony's little talk with her boyfriend, Noah. Meanwhile, AJ discovers a hidden talent.
Dr. Melfi decides to meet with her own psychiatrist to talk about her relationship with Tony. Later on, she is brutally attacked in an underground garage. Tony is tired of Janice's attitude, but is excited when he hears that his real estate project is is getting a huge amount of funding.
Bobby "Bacala" Baccilieri, Sr. comes out of retirement which makes Tony and Junior none too pleased. Adriana leaves her job at Nuovo Vesuvio after Christopher persuades her. Carmela decides to accompany Tony to a therapy session. Tony turns to a Newark assemblymant to "fix" a speeding ticket. Meadow accidentally ruins the FBI's wiretap in her basement.
Tony is unaware that Meadow and Noah's relationship has reached new levels, but Carmela has. Tony finds out that Ralph got one of the Bada Bing's dancers pregnant and that he beat her up.
Junior has some interesting dreams while undergoing stomach cancer surgery. Tony becomes enraged at Junior's doctor's attitude. Tony and Christopher continue their fighting. Carmela decides to attend Tony's therapy session when she discovers that Tony has been skipping them and soon Dr. Melfi refers her to another therapist for her own depression.
At Thanksgiving, Tony and Ralphie's feud begins to escalate after Ralphie is denied a promotion. Meadow begins to get closer to Jackie Aprile, Jr. Tony meets a woman while waiting for one of his sessions with Dr. Melfi, and soon begins to pursue a relationship with her.
Carmela is worried about Meadow's and Jackie's blossoming relationship, but Tony and Rosalie Aprile are fine with it. However, Carmela should be concerned about Tony's new relationship with Gloria Trillo. AJ and his friends broke into his high school and now must face the consequences. Meanwhile Christopher buys Adriana a nightclub.
The holidays are rapidly approaching, but Tony isn't feeling the Christmas spirit as he and Furio plan a retaliation against the Russian who beat up Janice. Meanwhile, as Tony, Sylvio and Paulie search for a new neighborhood Santa Claus, memories of Big Pussy haunt them. Lastly, Tony beats Jackie when he finds out that he cheated on Meadow.
Paulie and Christopher end up on a deadly chase with a Russian while out to pick up a collection. Tony is forced to deal with the crisis, but is torn between his family, business, and Gloria's fury. Meanwhile, Meadow and Jackie's relationship takes a turn for the worst when Meadow catches him with another woman.
Tony has to straighten things out when Gloria threatens to expose their affair to the Soprano family. Jackie, after getting some advice from Ralphie, attempts to pull a heist to prove himself, but when things don't go as planned, the heist turns deadly. Meanwhile, Carmela becomes interested in real estate.
After A.J. is expelled from school, Tony and Carmela fight over whether or not to send to him to military school. Meanwhile, Meadow thinks about her place as a Soprano and Ralphie makes a decision about Jackie.
Tony and Carmela both worry about family finances during the recession. Junior faces his own financial woes as his trial approaches. Adriana brings her new friend to the Soprano Sunday dinner, but Adriana has no idea who Danielle is working for. Tony bonds with Christopher by giving him the opportunity to avenge the murder of his father.
Meadow drops out of college to travel to Europe, which infuriates her parents. Trying to help her, Tony has Meadow talk to a psychologist, but it ends up doing more harm than good. Christopher is put in charge of Paulie's crew while Paulie is in prison, but it's soon discovered that Christopher isn't one for leadership. Meanwhile, Adriana is threatened by the Feds.
With Christopher running Paulie's crew, Paulie leaks information to Johnny Sack. Silvio and Ralphie go up in arms when they hear that a Native-American group is planning on protesting the Columbus Day Parade.
Tensions rise between Ralphie and Johnny Sack when Ralphie's comments towards Johnny's wife are made public. Tony orders a hit on Johnny and Johnny orders a hit on Ralphie. Meanwhile, Carmela realizes a new attraction towards Furio.
The FBI continues to follow Adriana in order to get information from her. Meanwhile, Carmela gets a stock tip and decides to invest $10,000, but Tony wants the cash to "invest" at the racetrack.
As Christopher continues to battle his heroin addiction, Tony gets a hold of information about Gloria Trillo. Meanwhile, Artie Bucco decides to kill himself after a bad financial decision.
After being in prison for four months, Paulie is released. The family throws him a welcome home party at Bada Bing where Adriana tries to get Christopher to marry her.
As Tony debates another affair, Paulie worries about his mother after she moves into Green Grove. Meanwhile, Furio tries to find a way to deal with his feelings for Carmela.
Uncle Junior stumbles across a loophole that would get him out of his trial. Meanwhile, Ralph becomes depressed when his son is injured; Tony sets out to seek revenge after the stable fire; and Christopher works with Tony to put together a disappearance.

All Johnny can offer is that "some homeless **** saw a guy limping away" from the murder scene. Soon after that Tony suffers an anxiety attack and collapses on the tee box.
