I'll start with Miles and Eva's case first. First, they head to one of the soldier's wife house for some interviewing. They didn't find much there and decide to test the community's pool and tap water. Since all the wives live in the same community, they share a lot of commonality. After the results of their test, everything came back clean. They then got a call that another wife had checked into the hospital due to contraction.

Miles calls Connors several times for what to do. Steven tells him to look around and Miles recalls that an ice cream truck had passed by several times. They tested the ice cream and found them infected (Don't asks me infected by what). Connor then treats his patients with antibiotics, but still finds one wife's baby with a low heartbeat. Miles decides that a c-section must be done to save the baby. The operation was successful.

Over in New York City, Connor and Co. investigate why one office worker was having a strange defect in their legs that paralyses them while at work. Her name is Meredith Beck. When another patient checks in, Eva now has the ability to find a commonality in their history. They were all exposed to the air near Ground Zero on 9/11. But when Dr. Durant asks Meredith on 9/11, Meredith freaks out and wants to avoid the topic. Natalie decides to take a step into Meredith's personal life, so she went into her house. There she found letters addressed to someone named, Annie. Natalie goes to Annie's house and finds out that Annie is dead. Annie was one of the unlucky ones that never made out of the Twin Towers during 9/11. Meredith bumped into Annie on her way down the tower and Annie was crippled on the stairs. Meredith promised that she will get help for her, but never did. Meredith regretted it all her life. But Natalie tells her that Annie's parents never blamed her and that she should just move on.

When Connor is ready to leave, Frank tells him that they are leaving without Natalie. Frank says she is at a hospital visiting someone. Connor tells Frank to move on without him too and went to Natalie to see what's going on. It turns out that Natalie had a similar patient like Meredith before, someone who is so depressed due to 9/11. Natalie never figured out the patient's depression and never quite cured her. Stephen comforts her telling that there's nothing she could've done and Dr. Natalie Durant is still one of the best in the business in the NIH.
